前言:世界衛生組織研究發現職業噪音引起的聽力受損的比率高達 16%。聽力檢查室目前有兩種方式,一是設置在工廠或醫院內;另一種是移動式的聽力車。依目前規定聽力車容許最大背景噪音量比先進國家有半數以上的頻率超出標準。因此,本研究想要了解聽力車做為聽力篩檢工具的可行性。
方法:在2008年收錄從事噪音工作的作業員共 98位。分別接受聽力車初檢為健康管理第三級者再接受醫院聽力檢查室來作最後確診。利用斯皮爾曼等級相關分析與魏克遜符號等級檢定方法來進行相關分析及檢定。
結果與討論:相關分析發現在左耳或右耳檢測兩者方法皆有線性相關。而魏克遜符號等級檢定方法結果發現除了左耳6000赫茲以及右耳三千赫茲與四千赫茲沒有達到統計上差異外(p>0.05),其他頻率皆有達到統計上的差異(p<0.05)。顯示兩種檢測方法可能有相當大的檢測差異存在。制定更嚴格的聽力檢查室可容許最大背景噪音量是可以改善兩者之間的差異性。所幸,最終管理等級評定兩者方法完全一致不會影響本國勞工的權力。
結論:本研究成果具有相當的參考價值可以提供職業醫學專家做為未來制定職業噪音檢查儀器標準設定及更嚴格的背景噪音容許值的參考依據,讓噪音操作員的聽力可以得到更多的保障。

英文摘要

Background: World Health Organization (WHO) studies conducted Occupational Noise-induced Hearing Loss composed of 16% of all types of hearing loss. There are two methods of audiometric test. One is the audiometric room established in hospital or factory. The other one is mobile audiometric apparatus. However, according to previous study, more than half of background noise frequency spectrums from mobile audiometric apparatus exceed the standard audiometric room. Our aim is to study the relationship between mobile audiometric apparatus and audiometric room, then, feasibility of using mobile audiometric apparatus as a hearing screening tool.
Method: We followed up 98 participants who were occupationally exposed to hazardous noise in 2008.These participants were evaluated by Mobile Audiometric Apparatus and confirmed by audiometric room. The correlation between two methods was calculated using Spearman Rank Correlation and Wilcoxon Signed-ranks Test.
Results: Spearman's Rank Correlation Coefficient conducted linear correlation between two methods including left or right ear. However, Wilcoxon Signed-ranks Test showed statistically significant in most frequency spectrums between two methods. The results showed there was statistically difference between two methods. But the result of final labor health management level of participants was coherent evaluated by the two methods.
Conclusions: According to our data, occupational health and safety specialists could establish the standard of Audiometric test environment and stricter background noise to improve the quality of mobile audiometric apparatus. It may protect more people who are occupationally exposed to hazardous noise.
