背景與目的:肩胛關節本體感覺對於肩部動作品質扮演著重要的角色。有學者認為肌內效貼紮能夠促進本體感覺,使動作正常化,避免再次受傷。先前已有文獻證實肌內效貼紮能夠改善肱盂關節本體感覺,而對於扮演穩定角色之肩胛關節則尚未有證據支持。因此本篇研究目的是要探討肌內效貼紮對於肩胛關節本體感覺、肩胛運動學及肌肉活性之影響。方法:本實驗收取30位健康受試者,將其隨機分配至控制組、肌內效貼紮組及偽貼紮組(3M透氣膠帶)。以測量關節復位誤差來評估關節位置覺,肩胛關節於肩胛上提(elevation)、下移(depression)、前突(protraction)及後縮(retraction)四個動作下測量。使用電磁式追蹤系統收集三度空間中關節位置及動作,以及表面肌電圖儀收集上斜方肌、下斜方肌及前鋸肌的肌肉活動。以二因子重複測量之變異數(two-way repeated measures analyses of variance, ANOVA)分析貼紮前後肩胛關節本體感覺的差異,肩胛運動學及肌肉活性則使用三因子重複測量之變異數分析。顯著水平α值定於0.05。結果:研究結果顯示,本體感覺部分,肌內效貼紮組於肩胛前突復位動作,肩胛內/外轉角度之復位誤差較另二組有顯著減少(p=0.004);肩胛運動學部分,肌內效貼紮組在舉手過程中較另二組有顯著肩胛後傾角度的增加(F=24.555, p<0.001),其餘變項則未有組間差異。結論:本研究證實肌內效貼紮能夠促進肩胛關節本體感覺且改變肩胛運動學,未來研究可針對肩痛患者貼紮,以觀察其臨床效益。

英文摘要

Background and purpose: Proprioception of the scapulothoracic joint plays an important role in movement quality of the shoulder complex. Taping has been shown to enhance scapular movement control, but its effect on the scapular proprioception is still unknown. Therefore, we designed this study to investigate the effect of kinesio taping on proprioception of scapulothoracic joint in healthy individuals. Methods: Thirty healthy subjects were recruited and randomly assigned into control, kinesio taping, or placebo taping group in this study. The scapular joint position sense (JPS) was measured as the reposition errors in the direction of scapular elevation, depression, protraction and retraction for the scapulothoracic joint. Liberty electromagnetic tracking system was used to collect the joint orientation and position. Surface electromyograhy was used to collect the muscle activity of upper trapezius (UT), lower trapezius (LT) and serratus anterior (SA). Two-way repeated measures analysis of variance was used to compare the joint reposition error of the scapulothoracic joint and three-way repeated measures analysis of variance was used to analyze scapular kinematics and muscle activity before and after taping. The level of significance was set at α=0.05. Results: Compared with the control and placebo groups, kinesio taping significantly decreased the reposition errors of the inward/outward rotation (p=0.004) when performing scapular protraction, and increased scapular posterior tilt (F=24.555, p<0.001) during scaption movement. Conclusion: Our study verified the effects of kinesio taping on scapular proprioception and kinematics. Further study is needed to research the benefit of kinesio taping for shoulder disorders.
