背 景:頸椎椎間盤突出症伴隨神經病變疼痛且持續一段很長的時間,影響其生活品質,然國內相關的研究相當少見。
目 的:探討頸椎椎間盤突出症病人神經病變疼痛、睡眠障礙與生活品質之相關性。
方 法:採橫斷式、相關性研究設計,方便取樣,收得有效問卷共102份,問卷內容包括人口學特性與疾病特性、以「日本骨科學會評估系統」評估疾病嚴重度、神經病變疼痛量表、中文維辛氏睡眠量表及臺灣簡明版世界衛生組織生活品質問卷等五部份,以SPSS 19.0版套裝軟體進行統計分析。
結 果:研究結果顯示,對象有中等以上的神經病變疼痛、中等程度以上的睡眠障礙,生活品質與疾病嚴重程度(p < .01)、神經病變疼痛(p < .05)、睡眠障礙(p < .01)均達顯著相關。疾病嚴重度和睡眠障礙可有意義的預測生活品質,共可解釋26.3%的總變異量。
結論/實務應用:頸椎椎間盤突出症病人生活品質受疾病嚴重度和睡眠障礙影響,建議若欲改善病人生活品質,可先由改善睡眠障礙著手,此結果亦可作為醫護人員後續照顧之參考依據。

英文摘要

Background: Cervical disc herniation (CDH) is associated with neuropathic pain that typically lasts for an extended period of time and significantly affects the quality of life (QOL) of CDH patients. Few studies of the effect CDH-related neuropathic pain on quality of life have been conducted in Taiwan.
Purpose: This study explores the relationship among neuropathic pain, sleep disorder, and quality of life in CDH patients.
Methods: This cross-sectional correlation study design used convenience sampling and collected 102 valid questionnaires. The 5 structured questionnaires used to collect data included a basic information sheet, the Japanese Orthopedic Association scoring system for disease severity survey, Neuropathic Pain Questionnaire, Verran and Snyder-Halpern Sleep Scale Chinese version, and the World Health Organization Quality of Life-BREF Taiwan version. Data were analyzed using SPSS for Windows 19.0 software.
Results: Participants reported a moderate level of neuropathic pain and sleep disorder. Disease severity (p < .01), neuropathic pain (p < .05), and sleep disorder (p < .01) correlated negatively with QOL. Disease severity and sleep disorder were significant predictors of QOL and explained about 26.3% of the total variance.
Conclusions / Implications for Practice: The most important predictors of QOL are disease severity and sleep disorder. This result suggests that reducing the incidence of sleep disorder among this patient population will improve their QOL. Study results provide important information for health professionals in caring for patients with cervical disc herniation.
