中文摘要

在台灣,根據衛生署的統計資料,從西元2002年開始,前列腺癌的發生率每年超過2000人,占台灣男性十大癌症第六位,男性癌症死亡率居第七位,已經超過膀胱癌,占泌尿系統惡性腫瘤中的第一名;近幾年,愈來愈多的研究顯示強度調控放射治療不僅可以明顯提高腫瘤控制率而且也降低鄰近膀胱或直腸的併發症機率,有效提升腫瘤患者的存活率。而目前最新之放射治療技術為體積調控弧形放射治療(Volumetric Modulated Arc Therapy, VMAT),它是一項先進之放射治療技術,結合強度調控放射治療 (Intensity modulated radiotherapy, IMRT)及影像導航放射治療技術(Image guided radiotherapy, IGRT),可有效增加腫瘤控制機率(Tumor control probability, TCP),同時降低正常組織副作用機率(Normal tissue complication probability, NTCP);因此在此放射治療技術下進行劑量驗證以維持治療品質是相當重要的。本研究使用直線加速器為ELEKTA 公司所研發之AXESSE加速器,其最大 照野為21×16 cm2,多葉式準直儀(multileaf collimator)寬度投射於機械等中心為4 mm;固態水假體30×30 cm2,將PTW Farmer type游離腔置於固態水假體中,射源距離測量中心軸為100 cm,增建厚度為5 cm、回散射厚度為10 cm;加速器輸出為100 MU並確認游離腔讀值之絕對劑量為1 cGy/MU,並且依腫瘤形狀多葉式準直儀會開啟順形照野;所使用假體為CIRS公司所出產之IMRT Head & Torso Freepoint Phantom,假體經由電腦斷層掃描後,將影像輸入Pinnacle3 smart arc 治療計畫系統進行計算,將其計算結果與游離腔及Gafchromic EBT 2 film進行比 較IMRT 與VMAT 劑量分布之差異。假體經由動態弧形放射技術照射後可即時得到游離腔之測量結果,而Gafchromic EBT 2 film經過十小時顯影後進行計讀,結果顯示治療計畫與游離腔之誤差為1~3%,而Gafchromic EBT 2 film與治療計畫之誤差Gamma function為95%。經由本研究可以建立動態弧形放射技術之劑量監測系統,可在病人治療前即可驗證所接受之處方劑量,達到放射治療劑量之品質保證。

英文摘要

In Taiwan, according to Health Department statistics, from 2002 AD, the incidence of prostate cancer more than 2,000 people each year, mortality rates have more than bladder cancer, malignant tumor in the urinary system accounted for the first. The latest radiation therapy technology for dynamic arc radiotherapy (Volumetric Modulated Arc Therapy, VMAT), which is an advanced radiation therapy technology, combined with IMRT (Intensity modulated radiotherapy, IMRT) and image guidance radiation therapy (Image guided radiotherapy, IGRT), can effectively increase the tumor control probability (Tumor control probability, TCP), while reducing normal tissue complication probability (Normal tissue complication probability, NTCP); so under this radiotherapy treatment dose verification in order to maintain Quality is very important. This study used ELEKTA AXESSE accelerator, the maximum field size is 21 × 16 cm2, multi-leaf collimator width was 4mm of the projection in the accelerator isocenter, and depending on tumor shape of multi-leaf collimator will open conformal field size; the use of the CIRS phantom company produced the IMRT Head & Torso Freepoint Phantom, phantom by CT scan, the image input Pinnacle3 smart arc treatment planning system to calculate its results and ionization chamber and Gafchromic EBT film for comparison. VMAT treatment planning compared with ionization chamber measurements, the point dose error of 1-3%. For 2D dose map, gafchromic EBT 2 film and Pinnacle3 Smart arc treatment planning results Gamma function is 95%. This study can be established through the VMAT technology. Before treatment can verify the accepted prescription dose to achieve quality assurance.
