本研究目的在探討脂締素濃度、胰島素阻抗與總膽固醇對高密度脂蛋白膽固醇比值對停經後婦女代謝症候群的預測性。採用橫斷式相關性研究設計,於北部某醫學中心內科門診及社區招募符合收案條件的停經後婦女,收集人口學屬性、身體組成與血液檢驗等資料,並以卡方檢定、獨立樣本t檢定、皮爾森相關分析、邏輯斯迴歸與複迴歸分析等統計方法進行資料分析。共78位停經後婦女參與本研究(平均年齡為56.91±6.23歲)。本研究結果發現多數婦女呈現體重過重、腹部肥胖(平均腰圍90.09±9.39cm)、空腹血糖值增高(平均104.87±13.56 mg/dL)與脂締素濃度降低(平均11.84±6.52 μg/ml)。而年齡、脂締素濃度、胰島素阻抗與總膽固醇對高密度脂蛋白膽固醇比值,可顯著預測停經後婦女代謝症候群,其解釋力達52%,其中以胰島素阻抗影響最大,脂締素濃度次之。停經後婦女發生代謝症候群的盛行率隨著人口老化有逐漸增加的情形,對停經後婦女代謝症候群可能的預測因子有更深瞭解,將有助於發展停經後婦女代謝症候群照護措施之參考,並提升停經後婦女的健康及預防心血管疾病合併症的發生。

英文摘要

The purposes of the study were to explore the metabolic syndrome (MS) risk predictors among adiponectin, insulin resistance, and total cholesterol to high density lipoprotein cholesterol (TC/HDL-C) ratio in postmenopausal women. Seventy-eight postmenopausal women completed this cross-section study in the cardiovascular department of a medical hospital center and community. Data of demography, body compositions and blood sampling were analyzed by chi-squared test, independent t-test, Pearson's correlation, logistic and multiple regression analysis. The postmenopausal women (mean age: 56.91± 6.23 years) in our study had overweight, central obesity (mean waist circumference: 90.09 ± 9.39 cm), higher fasting glucose levels (mean: 104.87±13.56 mg/dL), and declined adiponectin levels (mean: 11.84 ± 6.52 μg/ml). Age, adiponectin, insulin resistance, and TC/HDL-C ratio were significantly predicted MS in postmenopausal women (all p<0.01, R2=0.52). Results of our study supported the important roles of insulin resistance and adiponectin values in the development of MS, in addition to the components described by the National Cholesterol Education Program Adult Treatment Panel III (2001). Our study results are provided to the key instruments of healthy check-up and primary interventions to improve the health in postmenopausal women.
