物質成癮（addiction）意指個體對於成癮物質有強迫的需求，控制使用的能力失調。物質濫用者的治療可分為診斷、解毒、復健、長期追蹤四階段，解毒階段重點是安排個案短期住院藥物治療、協助緩解戒斷症狀，提供無毒的治療性環境。病患在解毒期常會因煩躁不安、渴癮等戒斷反應，繼而使用很多方式將成癮物質帶入病房。本專案目的是降低病患成癮物質持有率與異常事件發生率。分析原因：病患為了可解癮、現行安檢標準不合現況、無戒治動機、病患反社會性人格特質、安檢人員經驗與敏感度不足、家屬不認同戒菸規定、工作人員未確實安檢。透過緩解戒斷症狀措施、修訂安檢標準及人員教育、執行跨科室間交班規範、製作戒治佈告欄等措施。結果：病患成癮物質持有率從1.03%降至0.68%；異常事件發生率從0.26%降至0.08%。結論：本專案維持安全治療環境，降低成癮物質持有率及異常事件發生率，供其他物質成癮戒治單位參考。

The import of addictive substances into a ward by patients can result in patient intoxication, impairment of ability, interference with the treatment environment, increased stress for the medical staff, and interference with addiction treatment. Purpose: This study aims to decrease both the possession of addictive substances by patients and the incidence of accidents. Resolution: The study identifies (1) patients’ cravings, (2) unstandardized security checks, (3) low motivation for abstinence, (4) antisocial personality traits, (5) inexperience and lack of sensitivity among security staff, (6) family disapproval of the prohibition of cigarettes, and (7) incomplete security checks as sources of risk. In accordance with the identified sources of risk, many measures were undertaken in the ward, including the modification of treatment for the relief of withdrawal symptoms, the revision of security checks, the improvement of communications between staff in the many disciplines, staff education, and the provision of addiction treatment material in the form of a bulletin. Results: the rate of possession of addictive substances by patients dropped from 1.03% to 0.68% and the incidence of accidents dropped from 0.26% to 0.08%. Conclusions: This project achieves a secure and substance-free treatment environment and also provides a reference for other addiction treatment units.
