在醫學生的學習與養成中，如何習得有效應的(effectively)、有效率的 (efficiently)，以及人性的(humanely)評估(evaluate)與處理(manage)求治病人的 病程與預後，是首要的教學與學習目標，這當中，推理能力、批判能力與同情理解 的能力是最重要並需要培養的核心能力之一。在醫學診療整體行動的五個進程中 （anamnesis, diagnosis, prognosis, therapy and prevention），「診斷」階段的 邏輯框架最為複雜，也尤其需要邏輯推理以及批判性思維能力；另一方面，由於醫 學所面對的是生病與受苦的人體，換句話說，在生理上與心理上受苦的同類 (bio-psycho-moral agents)，它不只是一門科學(science)。因此，若有一門推論 技術是專屬於或可應用於人身上的生病現象的思維操作，則，它的對象勢必與傳統 形式邏輯這種專以命題集之間的「合乎形式」的推出關係，有著極大的不同；在醫 療場域中，醫師作為施治主體，病人作為受治主體，醫師面對「疾病」，除了「執行」 其醫學知識與醫療技巧之外，亦須「理解並尊重」病人的感受與期待。此外，在醫 療中，管理階層的權力結構，醫療團隊之間的專業倫理問題，醫療院所與主管機關、 健保體制以及醫療相關廠商之間的法律關連以及商業利益的精算等相關層面問題， 全部都以一定的程度在該次醫療中影響醫療決策以及診治行動的成功與否。這篇論文將嘗試在上述的邏輯框架下，觀察並反省醫學生的邏輯與批判性思考教學，其中，將以診斷階段的邏輯操作為主要的省察對象。在研究方法上，除了傳統論述分析法外，由於醫學領域中的思維操作一方面雖然是針對疾病的「過去成因」進行考察，然實則是面對一個未來諸多可以採取的行動進行一個「最佳未來」(preferablefuture)的選控，因此，在進行診斷階段的邏輯框架分析時，將使用「情境描繪法」(scenario)的未來學方法輔助分析。文章最後將嘗試指出，一個完善的醫學生與醫師的養成必須將邏輯與批判性思考能力的訓練納入正規的學習與訓練中。

In medical training, a crucial aim resides in how to determine the patients’ courses of disease, make accurate prognoses, and communicate with the patients efficiently and humanely. The author suggests that students' reasoning ability, critical thinking and sympathetic attitude are the most important elements and should be the focuses of training. Among the objects of medicine are diseases, bio-psycho-moral agents, and the institutions of medicine. On account of these, the inherent ethical-social networks are quite complicated. The author attempts to construct a suitable set of logical procedures (techné) to deal with these frameworks through examining case studies and configurating futurological clinical scenarios. At the end of this research, the author proposes a blueprint and typology of medical reasoning and modes of critical thinking within the webs of medical education.
