中文摘要

傷口裂開及骨外露是齒槽增高術之常見併發症,本病例報告提出改良式Edlan皮瓣以提高齒槽增高術之 成功。根據Edlan和Mejchar於1963年所提出的轉位皮瓣概念,本病例將傳統的三角皮瓣倒置,並同時取下頜骨 上升支區的自體骨做骨移植,等到骨頭與軟組織癒合穩定再做進一步植牙。由於使用改良式Edlan皮瓣設計, 傷口能在無張力狀況下縫合,於是傷口無裂開之狀況。此外,術後部的血液供應完好,牙種植體周圍的的角 化牙齦飽滿,和微量的骨吸收,整體結果令人滿意。(J. Family Dent. 9(4): 12-18, 2015)

英文摘要

Wound dehiscence and bone exposure are common complications in conventional three-cornered flap design and ridge augmentation for dental implants. This case report highlights the possible benefits of an inverted three-cornered flap design based on the transpositional flap described by Edlan and Mejchar in 1963. For dental implants, a modified Edlan flap procedure was used in combination with autogenous bone grafting from the mandibular ramus region. Implant placement was delayed until postoperative healing of bone and soft tissue was confirmed. Satisfactorily, dehiscence-free results were observed with regard to tension-free wound coverage using the modified Edlan procedure. Palatal blood supply remained intact after surgery. The keratinized gingiva around the dental implant remained adequate, and bone resorption from the ridge augmentation was minimal. The modified Edlan flap procedure may offer a simple and effective alternative technique for ridge augmentation. The flap design used in this procedure may reduce postoperative dehiscence, maintain keratinized gingiva, and minimize bone resorption. (J. Family Dent. 9(4): 12-18, 2015)
