齒瘤為常見的齒源性腫瘤之一,臨床分類可分為混合性(complex)及複合性(compound)齒瘤,其致病 原因尚無法確定,好發於兒童及年輕人。齒瘤多位於上下顎骨內,常無症狀合併生長緩慢之腫塊。腫瘤位置常 影響到鄰近恆牙萌發,須以放射線影像輔助診斷。本病例為一17歲男性患者,腫瘤位於上顎骨後上齒槽區,骨 頭向外推擠合併局部發炎感染。其直徑約3公分,藉由電腦斷層定位,在頰側面建立足夠通道,將腫瘤完整移 除,再藉由頰脂肪墊重建,經病理檢查為混合性齒瘤,患者傷口癒合良好,無併發症與復發跡象。(J. Family Dent. 9(4): 30-36, 2015)

英文摘要

Odontomas, which are the most common odontogenic tumors, comprise both epithelial and mesenchymal hard tissues. On the basis of gross, radiographic, and microscopic features, these tumors can be classified into the compound and complex types. The complex type is characterized by a hamartomatous lesion containing all dental tissues, with well-formed but unorganized hard tissue layers. These tumors are generally asymptomatic but are often associated with eruption abnormalities. Here we report the interesting and rare case of a 17-year-old boy who presented with a large complex odontoma in the left posterior maxilla that measured approximately 3.0 cm in diameter and caused impaction of the second molar. Computed tomography indicated an irregular, large, radiopaque mass in the left posterior maxilla. The tumor was surgically excised via the transoral approach under general nasotracheal anesthesia. Two weeks after surgery, the patient was in good health with adequate wound healing and no radiographic abnormalities. (J. Family Dent. 9(4): 30-36, 2015)
