張仲景傷寒論少陰病分循環性休克綜合徵及心臟衰竭兩大類。心臟衰竭分急性心臟衰竭，代償性心臟衰竭之附子湯證，及慢性心臟衰竭之真武湯證。對循環性休克而言，多數傷寒學者認為少陰病有寒化證與熱化證兩類，其實休克的生理機制反應是一種寒化的過程，但休克的病理機制，除寒化反應，亦會出現熱化反應，即過敏反應。少陰寒化證，包括四逆湯、白通湯、白通加豬膽汁湯、吳茱萸湯、附子湯、麻黃附子甘草湯、四逆散等證。少陰熱化證，有通脈四逆湯，麻黃附子細辛湯，桃花湯，豬膚湯、甘草湯、桔梗湯、半夏散及湯、苦酒湯，大承氣湯，豬苓湯，黃連阿膠湯等證。傷寒論對典型循環性休克的治療，當代償性休克進入嚴重休克，造成心臟血管退化的惡性循環，宜四逆湯急溫之以強心升壓。當嚴重休克併乳酸性酸中毒，引起身體及骨節疼痛，以附子湯抗休克、抗循環衰竭。當嚴重休克致肝組織壞死及呼吸性鹼中毒，心中煩不得臥，以黃連阿膠湯補充白蛋白養分，強肝解毒。若少陰病代償性休克，併發即發性過敏反應，引起下利，咽痛，咽瘡，不能語言，聲不出等，以豬膚湯、甘草湯、桔梗湯、半夏散及湯、苦酒湯等方治療。

The lesser yin disease in Shang-han lun by Zhang Zhong-Jing was classified circulation shock syndrome and heart failure disease. The heart failure disease was also classified acute heart failure, compensatory heart failure govern treated with Aconite Decoction and chronic heart failure govern treated with True Warrior Decoction. For circulation shock, most scholars of Shang-han lun thank that the lesser yin disease might classified cold forming pattern and heat forming pattern. In fact, the physiological mechanism of shock was a process of cold forming response. But the pathological mechanism of shock except cold forming response, it also might appear heat forming response called as an allergic response. The cold forming pattern in lesser yin disease included Counterflow Cold Decoction, Scallion Yang-Freeing Decoction, Scallion Yang-Freeing Decoction Plus Pig’s Bile, Evodia Decoction, Aconite Decoction, Ephedra, Aconite and Licorice Decoction, Counterflow Cold Powder etc. The heat forming pattern in lesser yin disease contained Vessel-Freeing Counterflow Cold Decoction, Ephedra, Aconite and Asarum Decoction, Peach Blossom Decoction, Zhu-fu Tang, Licorice Decoction, Platycodon Decoction, Pinellia Powder and Decoction, Vinegar Decoction, Major Qi-Coordinating Decoction, Polyporus Decoction, Coptis and Ass Hide Glue Decoction etc. The treatment of typical circulation shock in Shang-han lun, during compensatory shock change to serious shock which caused cardiovascular regression, it might strengthen the heart and elevated blood pressure treated with Counterflow Cold Decoction. When serious shock complicated lacto acidosis that caused body and bone-joint pain, it governs with Aconite Decoction which could anti-shock and anti-circulation failure. If serious shock complicated liver tissue necrosis and respiratory alkalosis caused mental insomnia, it governs with Coptis and Ass Hide Glue Decoction which supplied albumin nutrition and strengthen the liver and detoxification. If compensatory shock of the lesser yin disease complicated type Ⅰ allergic response which caused diarrhea, pharynx pain, pharynx ulcer, can’t talking, and hoarse, it governs formulas with Zhu-fu Tang, Licorice Decoction, Platycodon Decoction, Pinellia Powder and Decoction, Vinegar Decoction etc.
