本篇報告是描述一位因職業傷害，導致雙手壓碎性骨折接受多次清創、補皮及斷肢再 接手術病患的護理過程。照護期間為2008 年11 月12 日至2009 年1 月16 日，筆者與個案 建立良好護病信任關係後，運用溝通、會談、傾聽等技巧，以及Gordon 十一項功能性健康 型態評估，整合分析資料發現個案有疼痛、焦慮與身體心像紊亂等健康問題。筆者以主動 關懷、傾聽、同理心接納個案感受，針對上述護理問題擬定個別性護理照顧，以減少個案 焦慮、降低疼痛與負面想法、在情緒上獲得紓解，並運用醫療團隊及支持系統，協助個案 在創傷後心靈上的重建。期望藉由分享此篇護理照護經驗，提供護理人員在照顧類似個案 時，能兼顧疾病影響所及之身、心、靈適應，以作為往後照護之參考。

This report was about a nursing care experience of the patient who has a traumatic both hands injury at work. Unfortunately, this patient went through debridement, flap surgery, and replantation with microvascular surgery for several times. The period of nursing care was from November 12, 2008 to January 16, 2009. The patient’s data was collected and evaluated by means of direct caring, observation, medical reports and patient’s interview in physical, psychological, social, and spiritual fields. The patient’s health problems were pain, anxiety, body image disturbed. During the nursing care process, empathy, company, listening, and supporting were great methods to assist the patient to overcome his anxiety; furthermore, help him learn how to get along with pain, and to gain positive thoughts. This report provides a series of nursing care experience of a traumatic both hands injury with microvascular surgery and hopes these experiences will help other nurses in the futures
