許多研究顯示高壓氧治療可改善受傷骨骼的密度與強度,進而促進受損骨骼之修補, 臨床上也常應用於放射性骨壞死或慢性骨髓炎之治療,但高壓氧治療對於未受傷骨骼是否 也具有正向的作用至今仍未明確。本研究將透過分析臨床高壓氧治療之患者骨骼密度的變 化,初探高壓氧治療是否可用於改善骨密度,以期將來更進一步應用於骨質疏鬆症患者之 治療。我們搜集民國103年之7例病患(6例下肢慢性骨髓炎及1例放射性骨壞死), 其中男性3 例, 女性4例, 全部案例皆連續接受20次以上高壓氧治療,於進行高壓氧治療前與療程中第1 個月、第3個月分別進行股骨頸及腰椎(非患部)骨質密度的測量,高壓氧治療次數平均為 30.0 + 5.8次 (20 - 40次),測得治療前腰椎骨密度平均為1.181 + 0.079g/cm2 (1.061-1.268), 左股骨平均為0.847 + 0.126g/cm2 (0.672 -1.022),右股骨為0.862 + 0.131g/cm2 (0.685 -1.038)。第一個月骨密度平均變化量為腰椎: -0.006 + 0.063g/cm2 (-0.066 - 0.080),左股骨: 0.006 + 0.012g/cm2 (-0.006 - 0.024),右股骨: 0.007 + 0.024g/cm2 (-0.030 - 0.037);第三個月骨 密度平均變化量為腰椎: -0.037 + 0.047g/cm2 (-0.086 - 0.040),左股骨: 0.011 + 0.042g/cm2 (-0.053 - 0.067),右股骨: 0.004 + 0.019g/cm2 (-0.023 - 0.024)。本研究之結果顯示高壓氧氣治 療並未顯著影響本受試族群之骨密度,可能原因乃受限於樣本數較少。至於,高壓氧對於 骨質疏鬆患者是否有幫助則有待進一步的研究。

英文摘要

Many studies showed that hyperbaric oxygen (HBO) may improve the bone mineral density (BMD) and strength of the injuried bone and further enhance its repairment. Clinically, HBO therapy has been successfully used to treat the cases of osteoradionecrosis and chronic osteomyelitis. But the effect of HBO to the normal bone and the cases of osteoporosis remains unclear. This study is aimed to investigate the possible effect of HBO to the normal bone by studing the changes of BMD after the HBO therapy. In 2014, we collected seven cases ( six cases of chronic osteomyelitis of lower limbs, one case of mandibular osteoradionecrosis). There were three males and four females. All cases received the HBO therapy for more than 20 sessions. We measured the BMD of both femoral neck and lumbar spine for the uninjuried bone of the same patient before the HBO therapy , first month and third month of HBO therapy. The average sessions of HBO was 30.0 + 5.8 (20 - 40). Before the HBO therapy, the BMD of the lumbar spine was averagely 1.181 + 0.079g/cm2 (1.061-1.268),left femoral neck was 0.847 + 0.126g/cm2 (0.672 -1.022),and right femoral neck was 0.862 + 0.131g/cm2 (0.685 -1.038), respectively。The BMD measured at the first and the third month of HBO therapy was not significantly different as compared to that measured before the HBO therapy. The possible factor was due to the small sample size. Further study may be needed to realize the effect of HBO to the cases of osteoporosis.
