背景與目的:髖股骨關節炎(patellofemoral osteoarthritis)的重要性日前已經漸受重視。目前臨床指 引推薦使用非藥物策略作為處理關節炎症狀的第一線方法。此研究探討6°內側楔形鞋墊對於改善 髖股骨關節炎患者的症狀、髖股骨受力與壓力的影響。方法:14名主要問題為髖股骨關節炎的女 性受試者(年齡:68.8±9.3歲;身高:156.2±5.7公分;體重:64.5±11.2公斤)使用6。內側楔形 鞋墊後,觀察受試者在使用鞋墊前與使用後執行行走和下樓梯測試,並接受核磁共振檢查;藉由 八台ift速攝影機及動作分析儀器(Eagle CCD camera, Motion Analysis Corporation, Santa Rosa, CA, USA)與放射診斷用的臨床設備,分析疼痛程度、髖股骨受力與壓力的變化情形,實驗設計方式 採用橫斷面研究。結果:使用鞋墊後,受試者在下樓梯時的疼痛程度和髖股骨關節受力與壓力有 顯著性地降低,在行走時則無差異》討論與結論:6°內側楔形鞋墊能降低患者在下樓梯時髖股骨 關節受力與壓力情形,但行走時則沒有影響。然而,影響關節壓力減少的程度可能取決於動作困 難度或鞋墊的設計。後續研究需要確定內側楔形鞋墊的角度和哪種材料特性能產生較適當效果以 減少髖股骨關節的受力與壓力。

英文摘要

Awareness of the importance of patellofemoral osteoarthritis (PF OA) progression has been increasing. Current clinical guidelines recommend the use of non-pharmacologic strategies as the first line of management for OA symptoms. Purpose: This study investigated the influences of a 6° medially wedged insoles for improving the symptoms, PF joint force, and stress in individuals with PF OA. Methods: This is a cross-sectional study with fourteen female participants with predominant PF OA (age: 68. 8±9.3 years; body height: 156.2±5.7 cm; body weight: 64.5±11.2 kg). A six degree medial wedged insole was used as an intervention. The participants received a magnetic resonance imaging examination and were asked to perform walking and stair descent activities before and after insole application taken with the system of eight Eagle CCD (Motion Analysis Corporation, Santa Rosa, CA, USA). Results: After the insole application, the participants demonstrated significantly decreased pain level, PF joint force and stress during stair descent, but not during walking. Discussion & Conclusion: The 6° medially wedged insoles decreased the PF joint force and stress in participants with PF OA during stair descent, but not during walking. However, the extent of the effects of joint stress reduction may depend on difficulties of the tasks or the design of the insoles. Further studies need to determine which angle of the medial wedge and which material property produce the optimum effect on PF joint force and stress reduction.
