背景與目的:在高齡化社會,防跌訓練可以積極延緩老化預防失能,也是提升老年獨立生活能力的 重要策略。本研究目的欲探討多面向巡迴訓練對城鄉社區老人身體功能及動、靜態平衡之影響。方 法:本研究自台北、高雄及屏東徵召符合條件之社區老人,參與10周多面向巡迴訓練。訓練課程 包括防跌概念衛教及五大主題的多面向巡迴訓練 > 分別為伸展運動、心肺適能、肌力與耐力、平 衡與協調與敏捷力。以身體功能與動、靜態平衡檢測,評估個案訓練前後之成效。身體功能包含雙 手背後相扣 ' 坐姿體前彎、30秒坐站、兩分鐘踏步、2.44公尺繞物。動態平衡如富田踏步' 2.44 公尺繞物,靜態平衡如張眼及閉眼單腳站立。結果:共蒐集有效樣本118位’全體社區老人在下肢 肌力、心肺適能與敏捷力皆有顯著進步(P<.05),柔軟度僅右側有顯著進步(P<.05)。動態平衡項目 中2.44公尺繞物及富田踏步測試兩項目進步達顯著差異(^<.05 ),靜態平衡項目中張眼單腳站立時 間左右兩側進步皆達顯著差異(P<.05),閉眼單腳站立時間僅右側進步達顯著(p<.05)。城鄉社區老 人組間分析無顯著差異。結論:多面向巡迴訓練證實對城鄉社區老人身體功能與動、靜態平衡能力 提升有顯著成效,在運動設施缺乏之社區或一般老人居家防跌自我訓錬•其操作簡易且訓練器材易 取得,同時兼具安全與可近性,富有推廣價值。

英文摘要

Background and purpose: Fall prevention training was an important strategy for declined disability and enhanced the ability of the elderly to live independently. The purpose of this study was to evaluate the effects of multidimensional circuit training on physical function and balance ability in rural and urban for community-dwelling elderly. Methods: 118 participants from Taipei, Kaohsiung and Pingtung community-dwelling elderly were recruited to take 10 weeks multidimensional circuit training. The training programs include fall-prevention education and multidimensional circuit training which have five themes, namely, stretching exercises, cardiorespiratory fitness, muscular strength, balance and agility. Each participant was assessed for his/ her physical functions, static and dynamic balance abilities evaluation tools before and after intervention. Physical functions include flexibility, lower extremity muscle strength, cardiopulmonary fitness, and agility. Balancing ability include one leg stance test with eye open/close, 8-foot up and go test, and Fukuda stepping test. Results: The physical function of the participants were significantly improved in lower extremity muscle strength, cardiopulmonary fitness, and agility (p<.05). Flexibility was significantly improved only in the right side (p<.05). 8-foot up and go test and Fukuda stepping test were significantly improved (p<.05), static balance ability in the one leg stance test with eyes open were significantly improved (p<.05), but only the right leg significantly improved with eye closed (p<.05). Subgroup analysis by region, no significant difference was found in physical function and balancing ability. Conclusion: The multidimensional circuit training is proved to be beneficial for the community-dwelling elderly to improve their physical function and balancing ability. Multidimensional circuit training can be promoted for its simple and safe training equipment and operational procedure in both rural and urban community.
