本文為照顧一位韋格納氏肉芽腫疾病引發呼吸衰竭病患脫離呼吸器之護理經驗。筆者於2011年6月15日至6月27日護理期間以會談及觀察、翻閱病歷方式收集資料，運用來羅氏適應模式評估，確立有呼吸道清除功能失效、呼吸機戒斷反應功能失常、焦慮等護理問題。個案因疾病的藥物治療副作用導致抵抗力降低，進而發生感冒引發肺炎合併呼吸衰竭予以插管治療，除了面臨疾病引起的生理不適之外，也面臨脫離呼吸器之後的照護問題。於照護期間，先與個案建立良好護病關係後，引導個案及其家屬表達所面臨的照護困難，並提供疾病相關訊息，協助脫離呼吸器的個案轉至一般病房接受持續性的醫療照護，並與醫師、個管師及復健師等醫療相關人員，提供復健及出院後照護技巧，提升個案自我照護能力及家屬的參與和照護，終使個案病情改善順利出院。

A medical center in southern Taiwan had a high retention rate issue in their new physically and This paper describes the nursing experience of assisting a patient with respiratory failure caused by Wegener's granulomatosis to wean from the ventilator. From June 15 to June 27, 2011, data were collected through conversations, observations, and chart reviews based on the Roy's adaptation model. The health problems were identified as follows: ineffective airway clearance, dysfunctional ventilatory weaning response, and anxiety. The patient with lowered resistance to infection due to drug-induced side effects suffered from pneumonia complicated by respiratory failure due to a cold and was treated by intubation. The patient was faced with not only considerable physical discomfort attributed to the disease but also various care problems after weaning from the ventilator. During the nursing process, the authors established a good nurse-patient relationship, guided the patient and family members to express their difficulties in care, provided disease-related information, and then transferred the patient to a general ward for continuity of care after ventilator weaning. The authors also cooperated with other professional medical teams including physicians, case managers, and physiotherapists to provide rehabilitation and care skills after discharge, thereby improving the patient’s self-care abilities as well as family members’ participation and care skills and eventually assisting the patient in discharging from hospital smoothly.
