中文摘要

糖尿病(Diabetic mellitus,DM)會增加術後合併症及再發生率, 了解冠狀動脈繞道手術(Coronary artery bypass graft, CABG)是否 合併DM 自主管理的差異,以增進其自主管理,對病人長期生活 品質提升是非常重要的,本文即在於比較罹患DM 與否於CABG 後,病人自主管理情形與預測因子之差異。方法:採橫斷式方便 取樣,以北部三家醫院CABG 病人為對象,自2010.10~ 2011.3, 研究對象共350 位。研究工具包括基本資料問卷、自主管理、慢 性病自我效能及治療遵從性量表,以SPSS17.0 進行資料建檔與 分析。DM 組自主管理較無糖尿病組差;自我效能、治療遵從性、 收縮血壓及出現藥物副作用為DM 組自主管理之預測因子,自我 效能首先被納入,達38.5%解釋力,累積解釋自主管理59.4%變 異量(F=24.12, p<.001),無DM 組自我效能及治療遵從性可共同 預測自主管理,達33.1%之解釋力(F=44.82, p<.001)。 病人罹患DM 與否於CABG 後,其自主管理及其預測因子有 所差異,醫護人員可依照病人具有預測力之因子特異性,個別性 的加強病人自主管理,持續增進相關的健康行為,提升長期之生活品質。

英文摘要

Diabetes affects coronary artery disease mortality, Compare to non-diabetic patients, diabetic patients with CABG had more Complications and recurrence. To reduce complications and poor outcomes , identifying related post CABG risk factors of self-managment, figure out diabetic complications associated factors of better self-managment is essential on patient outcome for long term quality of life. The study to compare differences in predictors of self-management between diabetic and non diabetic patients who underwent coronary artery bypass surgery. Cross –section design with convenience sampling was use and 350 patients were recruited in three medical centers hospital. Instruments, included demographic questionnaire, self-management questionnaire, chronic disease self-efficacy questionnaire, compliance scale , were used. SPSS 17.0 for windows was used to analyze the data. The results showed non DM group, patients had higher self-management. In DM group, self efficacy, compliance, Systolic blood pressure, adverse drug reactions were the predictors of self-management, have 59.4% predictive power(F=24.12, p<.001).In non-DM group, self-efficacy and compliance were the predictors of self-management, have 33.1% predictive power(F=44.82, p<.001). The study results provided valuable information regarding diabetic impact on post CABG patients and the differences in predictors ofself-management between diabetic and non diabetic groups. The health care provider can promote healthy behavior and enhance quality of life through characteristics of the patients and factors associated with better self-management from the study results.
