背景與目的:探討利用電腦化適性測驗(CAT)評估日常生活活動功能量表之可行性。方法:利用 合併巴氏量表(BI)及芙蘭切活動量(FAI)的23及34題的兩種不同日常生活活動功能量表之試題 參數,模擬常態分佈1000受試者的兩種CAT生活活動功能分數,並與傳統全部作答比較其精準 度及效率性,且決定日常生活活動功能量表的決斷點。結果:全部作答的CAT量表,與其對應 全部作答的分數相關係數皆大於0.90,不相等的分數比率小於5%,答題數量節省74% ( = (34-9) /34)),決斷點的T分數(即平均值為50,標準差為10)設定於45、55、與65分。結論與臨床意義: 利用CAT評估日常生活活動功能量表會比全部作答更有效率,卻不失其測量的精準度。臨床工作 者可利用智慧型手機進行線上日常生活活動功能量表的評估。

英文摘要

Background and Purpose: The aim of this study is to investigate the feasibility of using computerized adaptive testing (CAT) to measure the activities of daily living (ADL) function. Methods: We retrieved item parameters of the combined Barthel Index (BI) and the Frenchay Activities Index (FAI) from literature, i.e., 23-and 34-item scales, and used a normal standard distribution to simulate CAT and Non Adaptive Testing (NAT) on answers of 1000 subjects. Pearson correlation coefficients and the ratio of inconsistency between the CAT and NAT responses were calculated, and the cut-off points for the scale were computed and determined. Results: The results showed that the correlation coefficients between the three CAT estimated measures for its NAT counterparts were greater than 0.90. There was less than 5% inconsistency between CAT and NAT responses. The number of questions required for the CAT was 74% (=(34-9)/34) less than the NAT. The cut-off points denoted by the T-score were 45, 55, and 65. Conclusion and Clinical Relevance: CAT was more efficient than NAT without compromising its precision for the assessment of ADL function. We suggested that clinicians could use online CAT with smart phones assessing ADL function.
