本專案主旨是提升外科加護病房護理人員執行連續性血液 過濾機正確率。護理人員若對儀器操作不正確，不僅在照護過程 中增加成本，更易危害病人安全，專案小組於2012 年8 月發現 護理人員操作血液過濾機技能正確率僅達54.5%，對於機器照護 知能正確率只達62.7%。主要原因為無操作規範及操作流程指引、 照護認知不足、備物及儀器操作不熟練、缺乏在職教育、無練習 教具、說明書內容繁雜。經小組制定標準護理技術、舉辦教育課 程、統整用物列表、設立物品專用櫃、設置練習教具、製作教學 影片及操作圖片手冊等措施，技能正確率由54.5%提升至92.7%； 知能正確率由62.7%提升至94.9%；透過專案介入，有效提升護 理師操作儀器正確率，平行推廣給其他單位參考，共同提升病患 照護品質及安全。

The instrument operation were not correct will effect endanger patients life, With just 54.5 % of the operator operation instrument integrity from August 2012, and for the operation technical proficiency of machine correct rate only 62.6%. The analyzed reasons were: no standard operation procedure (SOP)、different cognitions in the instrument、operator not proficiency handle instrument、not enough training of instrument 、no training tools 、documentation to complicated of instrument. Via unit group set up SOP of nursing technology 、educate program、list all component、set up cabinet of component 、set up training teaching aid and video with operation document, the correct of operation skill 92.7% was achieved; knowledge skill 94.9% was achieved; The project is helpful to enable the nurses to obtain the correct operation instrument and improve correct rate, will promotion to other department for reference, will improve patients care quality and safety.
