本文描述一位小腸切除術後個案與家屬於加護病房，面對疾 病及環境壓力下，個案出現身心問題，家屬對醫護照護出現不信 任感，於會客時間有拍照、錄音之行為，故引發筆者針對此個案 及家屬探討之動機。護理期間為2012 年4 月3 至4 月19 日，筆 者以觀察、病歷查閱、身體評估、與家屬會談等方式收集資料， 運用 Gordon 十一項健康功能型態進行評估，分析歸納個案主要 之護理健康問題為「低效性呼吸型態」、「急性疼痛」、「焦慮」； 家屬方面有「焦慮」及「預期性哀傷」之護理健康問題。於護理 過程中運用全人照護建立良好護病關係，提供輔助療法(廣播、喜 愛物品─帽子、按摩、放鬆技巧等)，減緩個案身、心理不適，予 同理心、主動關懷及耐心解釋，了解個案及家屬需求，使得醫、 護、病關係改善，進而協助個案脫離急性期，並順利轉至普通病 房續治療。希望藉此照護經驗與大家分享，並提供日後護理人員 照護類似個案之參考。

This report describes the experience of nursing a small bowel resection patient and family members in ICU. Under the pressures of illness and environment, the patient exhibited physical and mental issues. Family members showed distrust of the medical care, hence, taking pictures and recording during visiting hours. During the nursing process, from April, 3rd 2012 to April, 19th 2012, the writer collected information through observation, chart review, body health evaluation, and interview with family members. Gordon’s 11-item Functional Health Patterns was applied to evaluate the patient’s health condition, and several nursing problems were observed in this case: ineffective breathing pattern, acute pain, and anxiety. Other nursing problems on the family side included anxiety and anticipatory grieving. The writer established a therapeutic nurse-patient relationship by using holistic health care, and provided complementary therapy (such as radio, adoring objects – hats, massages, relaxation techniques, etc.), in order to ease the patient’s physical and mental discomfort. The writer also showed empathy and patience during the nursing process, actively understanding patient and family members’ needs. The nurse-patient relationship was then improved, and the patient passed the critical stage and was transferred to general ward. The writer hopes that this nursing experience can serve as a reference for other nurses that encounter similar cases.
