本文描述一位25歲女性因紅斑性狼瘡反覆感染誘發右足踩骨髓炎住院接受治療的照護經 驗，護理期間為2011年8月25曰至10月11曰。筆者運用Gordon十一項功能性健康型態為 評估工具，利用身體評估、觀察及會談等技巧收集資料，發現個案有組織完整性受損、急性疼 痛及不確定感等健康問題。筆者先與個案及家屬建立良好治療性關係，運用護理知識及結合醫 療團隊力量追蹤個案骨髓炎的病情變化、緩解個案疼痛，提供傷口照護之相關衛教，傾聽、接 納並鼓勵個案說出疾病感受、自身想法，並對其參與治療計晝給予正向鼓勵，減輕個案之不確 定感，使其獲得生理症狀緩解與維持心理舒適，期望此個案照護經驗能提供臨床護理人員參考。

This article described nursing experiences in a 25 year-old female suffering from systemic lupus erythematosus (SLE) with right ankle osteomyelitis due to repeating infection. The period of nursing care was from August 25 to October 11 in year 2011. The author used physical assessment, observation and interview to gather information and analyzed these data with Gordon’s 11 functional health pattern models. The major problems were identified: impaired tissue integrity, pain and uncertainty. The author established a good therapeutic relationship with client and her family first. Then the author applied nursing knowledge and cooperated with medical team to follow the condition of osteomyelitis, release the pain and teach skills of wound care. To assist patient to eliminate uncertainty, the author listened and encouraged the patient to express the thoughts about the illness and treatment plans. Finally the patient could relief the symptoms of physiology and maintain the comfort of psychology. We expected the caring experiences could be the reference for clinical nurses.
