手指遭機具壓砸傷對工廠作業員來說極為常見，即使手指癒合能力強，當傷口缺損過大或過深造成底下關節軟骨，肌腱或骨頭暴露時，若無積極手術處理將會導後續傷口潰瘍感染，關節僵硬，甚至截肢的命運。本研究報告一位年輕男性因工作受傷致大拇指背側大面積皮膚與軟組織缺損，施行第一背側掌骨動脈皮瓣重建手術後，不僅傷口順利癒合，大拇指的重要運動及感覺功能亦順利恢復，得以再度回到工作崗位上。

Finger crush injuries caused by machine are very common for factory workers. Despite the strong healing power of finger, serious complications such as chronic ulcer, infection, stiffness or even amputation will encounter if the wounds were too large or deep that the underlying articular cartilage, tendon or bone was exposed without adequate surgical intervention. This study report a young worker sustaining dorsal thumb large skin and soft tissue defect after trauma. After reconstruction surgery with first dorsal metacarpal artery flap, not only the wound healed uneventfully but also the important motor and sensory functions recovered well that finally the patient can back to work without problems.
