跨領域團隊合作照護為近年來醫學照護品質中相當重要的一門課題，多數患者在接受醫療照護上，皆由多名 跨職類醫護人員提供專業與協助。放射治療領域多為跨領域團隊合作照護，其中各類專業人員包括西醫(放射腫 瘤科專科醫師)、護理師、醫學物理師、醫事放射師、個案管理師、管理師…等，若當缺乏團隊合作的溝通與默 契，易局限於各自之專業領域，無法以病人為中心發揮最大團隊醫療效益。本研究以腦癌患者為例，藉由整合之 放射治療電子化系統，連結各領域之專業所長，以提升團隊照顧之效率。結果顯示系統化的整合評估流程有效提 升病人安全、臨床異常事件大幅降低，檢視病患綜合滿意度調查各面向平均皆高達4.8 分(滿意為4 分、非常滿意 為5 分)，99%患者皆表示滿意(4 分以上)之等級，遠大於本科品管中心指標85%。由此可見，藉由跨領域團隊合 作共同照護，確實能培養組織間溝通協調的默契，並透過強化系統與整合流程，啟動有效的合作照護，有效提升 病人安全及醫療品質。

During recent years, Inter-Professional Practice (IPP) has become one of the important practices in medical care quality. Most patients in healthcare require multiple branches of medical experts to treat and care for diseases. The majority of radiotherapy patients are among those in need of cross-field expertise, such as oncologist, medical physicist, radiologist, nurse practitioner, health record manager and case manager. Through improved communication and understanding in the healthcare teamwork can reduce medical error and increase team efficiency. Taking this brain cancer case as an example, by integrating electronic system of radiotherapy and collaborating expertise in different field to improve healthcare efficiency. The results have proven that systematic evaluation process has greatly improved the patient safety, no clinical abnormal case occurred. The average score of patient’s satisfaction survey in each perspective was reached 4.8 out of 5 as close to very satisfied status, 99% patients considered as 4 or above which is exceeded the quality assurance center’s 85% target. Thus, it’s proven that IPP can bring up the inter-groups’ communication; meanwhile, improving the patient safety and healthcare quality by enhancing electronic system and integrating standard operation process (SOP).
