本文係探討一位大腸癌術後病人首次面對化學治療之護理經驗，護理期間於2011年8月 8日至8月14日運用直接照護、觀察、會談、傾聽、收集病史及Gordon十一項健康功能型態 進行整體性評估，並確立護理問題包括：噁心、焦慮、潛在危險性感染、照顧者角色緊張。筆 者於照護期間以關懷及同理心的態度與個案及家屬建立良好的護病關係，提供自我照顧的衛 教、副作用評估及處置，來降低對治療的不確定感，增進對化學治療的準備度，並將照顧者納 入為照護對象，以減輕焦慮，出院時發現個案有將護理人員提供護理指導運用在生活中，例如： 使用檸檬水對抗噁心感，且嘔吐後有立即執行口腔護理，來改善噁心、嘔吐不適之情形，出院 後以電話諮詢追蹤，個案表示已無嘔吐情形，並能遵守衛教内容，且繼續與個案共同參與治療 過程，協助面對接受首次化學治療。期望藉此護理經驗可提供臨床護理人員協助此類個案對化 學治療的正確認識及面對化學治療的副作用以提升自我照顧能力。

This essay investigates a patient faces to the chemical treatment after colorectal cancer. The period of nursing and care was August 8th to August 14th. By using caring, observing, talking, listening and collecting medical history and evaluating by Gordon 11 functional health patterns, confirmed the nursing question included nausea, anxiety, potential dangerous infection and nervousness of nursing character. With care and empathy, I built good relationship with patient and family. Providing them Instructions of self-caring, side effect evaluation and solutions to decrease the uncertainty in illness, had better preparation on chemical treatment. Also, the primary nurse treat nursing character as my patient to ease their anxiety. After discharged, the patient had used nursing instruction which was being taught by nursing staff in daily life. For example, using lemon water to reduce nausea then executed mouth care to improve nauseated and vomiting. After patient discharged, the primary nurse used telephone consultation to follow up the situation of patient. The patient said that there was no vomiting symptom and he can follow the content of nursing instruction. Also, the primary nurse will continue the process of treatment to help the patient to face the first time of chemical treatment. Thus, the primary nurse expect that can provide clinic nursing staff this experience to have positive knowledge to help patient to face chemical treatment and side effect.
