初期血液透析病人於開始透析期間得到完整護理指導可增加自我照護知識，降低 面臨長期透析焦慮、衝擊，於返家後能有好的自我照顧能力，因此激發此專案改善動 機。本專案旨在提升初期血液透析病人護理指導之成效，專案期間自 2015 年 1 月 01 日∼2015 年 11 月 30 日，經調查、分析及文獻查證後，選定六項改善方案：訂定初 期血液透析病人護理指導標準流程規範、持續品質監測、以創新手法製作書面輔助工 具及多媒體影片、改善護理指導環境、擬定定期在職教育訓練課程，專案推行後主要 結果發現：護理指導完整率由 57.1％提升至 96.92％，病人的滿意度由 48％提升至 88.5％，故本專案除提升護理指導完整率亦提升病人滿意度，對護理品質具正面之肯 定。

The provision of comprehensive nursing instructions for new hemodialysis patients can enhance self-care knowledge, decrease long-term anxiety, and improve their self-care ability after returning home. Our project is therefore aimed at enhancing the effectiveness of the nursing guidance for new hemodialysis patients. Between January 1, 2015 to November 30, 2015, six improvement programs were selected after studying, analyzing and verifying the relevant literature, including: Establishing a standard operating process on nursing guidelines for new hemodialysis patients, continuous quality monitoring, preparation of innovative written aids and multimedia videos, improving the nursing instruction environment, as well as the development of regular on-the-job education and training courses. After the project was implemented, nursing instruction integrity rate improved from 57.1% to 96.92% and patient satisfaction increased from 48% to 88.5%. The results suggest that our project not only enhanced the effectiveness of nursing instruction for new hemodialysis patients but the quality of nursing care.
