一位43 歲男性接受經皮冠狀動脈介入治療,從右冠狀動脈近端到遠端分別置放3.5×23 mm、3.5×28 mm 和 3.0×28 mm 的生物可吸收模架。術後18 個月的追蹤,再次冠狀動脈攝影發現模架置放的位置產生了血管瘤。光學 同調斷層掃瞄發現模架的結構已呈現非連續狀態,且血管直徑與管腔面積均有增加。生物可吸收模架是冠狀動脈 介入治療的全新應用,但冠狀動脈血管瘤發生的病理機轉目前仍是未知。期盼未來能有更多研究揭示防止血管瘤 發生的最適化策略。
A 43-year-old man underwent percutaneous coronary intervention from the proximal to distal right coronary artery with 3.5×23 mm, 3.5×28 mm and 3.0×28 mm bioresorbable vascular scaffold (BVS) successfully. At 18-month, follow-up coronary angiography showed coronary artery aneurysm (CAA) formation over the BVS. Optical coherence tomography showed BVS strut discontinuity and increased the vessel diameter and lumen area. BVS is the brand-new device in percutaneous coronary intervention, but the pathological mechanism of CAA is still unknown. We hope the future studies will reveal the optimal strategy of blocking CAA formation.
