前言:慢性腎臟病在世界上的盛行率逐年上升,且有多項風險因子包含高血壓、糖尿 病、高血脂、與高尿酸血症等。醫療從業人員因為生活型態可能更易導致上述慢性疾病的 發生,因此欲藉由分析醫院員工健康檢查的結果,探討風險因子與腎功能之間的關係。 方法:本研究為橫斷面的研究,連續性變項以平均數(+/-標準差)來表示。推論性統計 方面則以獨立樣本T檢定和卡方檢定來分析男女兩組間的差異性。進一步利用單變項與多變 項線性回歸分析求得何者是腎功能下降的獨立危險因子。 結果與討論:本研究收錄民國104年至105年在某醫院體檢的員工共849人,結果發現年 齡、肥胖、高血壓、高膽固醇血症、高三酸甘油脂血症、及高尿酸血症都與腎功能下降相 關。進一步以多變項線性回歸分析,發現影響預估之腎絲球過濾率降低的因素包含年齡 (B=-0.84, 95% CI -1.05~-0.63)、膽固醇總量(B=-0.06, 95% CI -0.12~-0.01)、尿酸(B=-7.90, 95% CI -9.65~-6.15)與預估之腎絲球過濾率呈線性負相關。 結論:年紀,總膽固醇,血中尿酸值與腎絲球過濾率預測值呈現負相關。

英文摘要

Background and objective: The prevalence of chronic kidney disease increased gradually around the world. There are multiple risk factors contributing chronic kidney disease, such as hypertension, diabetes mellitus, dyslipidemia, and hyperuricemia. Health-care employees in the hospital are more prone to acquired chronic disease due to poor life style, so we would like to analyze the outcome of health exam among hospital employees to revealed the association between renal function and its risk factors. Method: This is a cross-sectional study. The laboratory and anthropometric data from occupational health examination in 2015~2016 in a hospital in Taiwan were extracted. The continuous variables present with mean (+/- standard deviation). We used independent-samples T test to compare the demographic and clinical characteristics between male group and female group. Finally, univariate and multivariate linear regression analysis were used to find independent risk factors. Results and discussion: 849 healthy workers were enrolled in one hospital during 2015~2016. Our study revealed that age, BMI, hypertension, hypercholesterolemia, hypertriglyceridemia, hyperuricemia were all related to the decline of eGFR level. Through multiple linear regression analysis, we revealed that eGFR level was negatively associated with age(B=-0.84, 95% CI -1.05~-0.63), total cholesterol(B=-0.06, 95% CI -0.12~-0.01), and uric acid(B=-7.90, 95% CI -9.65~-6.15). Conclusion: Age, total cholesterol, and uric acid were negatively associated with eGFR level.
