本文運用娛樂活動於照顧一位支氣管腫瘤合併慢性呼吸衰竭需長期依賴呼吸器維生病 患,因病房感控需求異動其床位遠離護理站,導致個案對環境產生不安全感及疾病導致造成身 體不適,而引發焦慮情緒。2015年9月10日至2015年10月30日期間,筆者以支氣管腫瘤 及慢性呼吸衰竭的護理概念,運用生理、心理、社會與靈性四層面的評估方式,進行收集資料, 確立個案有營養少於身體所需、呼吸道清除功能失效、娛樂活動缺失及焦慮等健康問題。透過 傾聽、關懷與其建立互信的護病關係,教導於協助抽痰時利用腹壓將痰液咳出氣切套管外,瞭 解個案喜好的休閒方式,設計適切的娛樂活動,藉由參與增加個案的自信心,此外,應用家庭 支持系統及提供喜好的音樂介入,有效緩解焦慮情緒及改善住院期間的生活品質。

英文摘要

In this study, recreational activities were presented to take care of a patient with bronchial tumor combined with chronic respiratory failure depending on life-long respirators for a living. Patients’ beds of respiratory care ward were move away from the nursing station due to the need of infection control. Therefore, the patient developed anxiety emotions because of the sense of insecurity from an unfamiliar environment. Based on the nursing concept, the patients’ data were collected from physiological, psychological, social and spiritual observations between September 10, 2015 and October 30, 2015. The health issues of the patient were identified as nutrition imbalanced less than body requirements, respiratory tract clearance function impairment, lack of recreational activities and anxiety through an overall nursing assessment. A mutual nurse-patient trustful relationship was established through listening and caring. As a result, nursing professionals could teach the patient to cough up sputum out of the tracheostomy tube with the assistance of abdominal pressure during suction. Furthermore, nursing staffs were able to increase patient’s confidence by understanding patients’ leisure preferences and thus designed proper recreational activities for patients to participate. In addition, family support system and the intervention of favorite music also effectively relieved anxiety emotions and improved quality of life during hospitalization.
