本文為探討一位68歲女性腦中風病人，於住院期間面對腦中風後肢體障礙及心理衝擊過程之 護理經驗。照護期間自2014年5月4日至5月20日，以觀察、會談、家屬訪談、身體評估、 查閱病歷等方式進行資料蒐集，護理過程運用歐倫自我照顧理論進行整體性的評估，確立個案 有皮膚完整性受損、身體活動功能障礙、自我照顧能力缺失(穿衣、進食、洗澡、如廁）、無力 感之護理問題。筆者以關懷及陪伴等方式與個案、主要照顧者建立良好護病關係，運用歐倫理 論的完全代償性、部分代償性活動及支持教育性護理等概念提供病人個別性的照護，壓瘡傷口 之護理照護，以減少併發症的產生；掌握關鍵黃金復健期進行肢體復健，以改善肢體活動功能； 教導及加強病人日常生活中自我照顧技巧；採主動關懷支持、積極傾聽，鼓勵病人表達内心感 受及給予心理支持減輕無力感，進而改善生活品質。

This paper is a description of the nursing experience to a 68 years old stroke patient who suffered from physical immobility and psychological impact of handicap. Using Orem5s self-care theory method, the author collected the data through careful observation, phone conversation, interview, family visit, patient’s physical assessment and reviewing of patient’s medical records that started from May 4 to May 20 in 2014. This particular patient suffered from pressure sore, disability, ADL dependent and depression. Author established a good nurse-patient relationship through professional care and frequent visit.The patient has been taught the skills of self-care and an aggressive rehabilitation therapy was also given . The patient has also been encouraged to express his feeling, seeking psychological support to promote quality of life.
