本文敘述一位31歲已婚女性,因父親罹患肝癌末期需接受肝臟移植延續生命,故入 院預行活體肝臟捐贈,但個案入院後卻因案夫與婆婆反對,經由召開家庭會議達成捐贈 者和家屬醫療決策共識,使手術能順利進行。護理期間為2015/9/10至9/25,筆者運用 Gordon十一項功能性健康型態評估,藉由觀察、會談等方式,確立個案有捐贈者及家 屬間醫療抉擇不一致/親情羈絆與決定時間侷促有關;急性疼痛/與手術傷口、腹脹、引 流管牽扯有關;活體肝臟捐贈者術後自我照顧衛教指導需求等三個健康問題。照護期間 透過與主要支持者討論、運用成功案例分享與宗教關懷師的介入,使個案與家屬對手術 有一致看法並針對擔心事項提出實際可行之方法。希望藉此照護經驗提供護理同仁,面 對親友阻抗移植的個案時,需深入了解捐贈者捐肝手術前後身心的變化及支持系統,提 供適切的個別性完善照護,以促進活體器官移植之照護品質。

英文摘要

This article describe the nursing care of a 31-year-old married female, whose father was a terminal case of liver cancer. She was admitted as living donor for liver transplantation. However, due to the opposition of her husband and mother-in-law, the surgery was delayed. The nursing care started from September 10, 2015 to September 25, 2015. By using Gordon eleven functional health assessment guideline, we found there were three main problems through observation, physical assessment and other medical records. The first one is : difficulty in decision making and doesn’t have enough time to think about. The second one is: acute pain, which may be related to operation wound pain, abdominal fullness and the traction of drainage tube. The third one is post operation self-care and heath education. During this nursing care period, by discussing with the key person, sharing successful cases and through religious intervention, we provided feasible ways to manage their worries. We hope through this nursing experience, we can help others understand that we should dig deeper into the donor’s peri-operative mental change and supportive system. In order to provide more appropriate care and to improve the nursing quality of living donor liver transplantation.
