本研究旨在探討失智症長輩在機構式照顧中,工作者提供節慶懷舊團體之非藥物性處遇模式之成效。研究方法採取準實驗設計法,規劃節慶懷舊組與一般未參與團體服務之對照組,進行探討兩組成員MMSE 智能分數及憂鬱傾向量表前後差異之情形。實驗場域於彰化地區某長期照顧機構,研究對象邀請以簡易智能狀態檢查(MMSE)量表及憂鬱傾向量表篩選出失智住民,以邀請其參與團體。團體進行方式,採半開放性團體,每週進行團體二次,總計進行八次團體單元進行、每次九十分鐘。共14 位長輩參與節慶懷舊團體處遇服務;另外,亦邀請個人身心狀況相似的12 位一般長輩作為對照組。研究結果顯示:1.節慶懷舊團體對輕度及中度失智症長輩的憂鬱傾向有顯著的改善或降低憂鬱傾向,此與其他懷舊主題的團體處遇有相同的結果。2.一般長輩的確在其憂鬱傾向的分數未有明顯的改善;因此,經本準實驗驗證下,節慶式的懷舊團體對改善憂鬱傾向的輕度及中度的失智症長輩有其顯著效果。3.短期來看此非藥物性處遇團體,對於改善或減緩失智症病程並無顯著效果。4.七成以上長輩分別在參與節慶懷舊團體處遇服務後,在憂鬱傾向的分數有所降低;且有五成以上的長輩原本具有憂鬱傾向(達五分以上),在團體結束後皆降低至五分以下。研究建議為:1.工作者需依據客觀的需求調查了解長者的基本屬性等作初步的邀請;進而,運用適當且簡易心智能力評估工具做整體性的評估,以利籌畫合適之懷舊主體做為實務團體處遇。2.對已確診的輕度或中度之失智長輩,可直接對其懷舊主題做評估,以避免重複檢測。3.對於疑似失智症之長者,可以先評估其懷舊主題及基本資料後,佐以簡易量表做客觀的失智症狀篩選。在機構或社區照顧中可藉由此型態的懷舊團體作為篩選的機制,以利早期發現。4.懷舊主題可以多元研發,例如可以結合輔具或其他健康促進方案之主題(如穿戴裝置、運動、歌唱、旅遊等)戶外活動型態辦理。5.對於本類型及其他類型之失智症狀的改善並無法得知,未來可參照其他研究報告,進一步蒐集針對個別長輩的認知、行為,或其他相關資料做分析。

英文摘要

The purpose of this study was to explore the effectiveness of the nostalgic festival story group for mild and moderate elderly dementia patients. The research method adopted the quasi-experimental design method and planned the nostalgic festival story group and a control group. The pretest and the posttest of the two groups were measured by the MMSE intelligence score and the depression tendency scale. The experiment was conducted in a long-term care institution in Changhua. The researchers applied the Simple Intelligence Status Examination (MMSE) Scale and the Depression Tendency Scale to select the mild and moderate elderly dementia patients as the participants of the group. It was conducted twice a week, 90 minutes per session for eight times. A total of 14 elders participated in the nostalgic festival story group. Besides, 12 ordinary elders with similar physical and mental conditions were invited to be in the control group. The results of the study showed that: 1. The nostalgic festival story group had a significant effect on the depression tendency of mild and moderate elderly dementia patients, which had similar results with other types of nostalgic groups. 2. The average elderly participants’ scores of depression did not significantly improve. This result indicated that the nostalgic festival story group had a significant effect on the level of depression of mild and moderate elderly dementia patients. 3. In the short term, this non-drug treatment had no significant effect on the course of dementia. 4. More than 70% of the elders participated in the nostalgic festival story group, the level of depression decreased; more than 50% of the elders with a higher level of depression (more than five points), the level descend to five points or less after participating the nostalgic festival story group. Suggestions: 1. Objective needs surveys should be conducted before inviting the elders; further, use appropriate and simple assessment tools to make a holistic assessment to facilitate the selection of suitable nostalgic subjects. 2. For those who have been diagnosed as mild or moderate dementia patients, workers will only have to conduct an assessment for the proper nostalgic subjects instead of a holistic assessment. 3. In order to facilitate early detection for the actual patients of dementia and assist the long-term care service providers such as institutions or local caregivers within the community units, the evaluation on elders’ nostalgic themes, the collection of their basic information and objective assessment of their level of dementia symptoms will be needed. In the institutional or community care, this type of nostalgic group can be used as a screening mechanism to facilitate early detection. 4. Nostalgic themes can be developed in multiple ways, for example, in combination with the theme of accessories or other health promotion programs (such as wearing devices, sporting, singing, traveling, etc.). 5. Ways to improve the level of dementia are still unknown. In the future, other research reports may be referred to further analyze the cognition, behavior, or other relevant information of individual elderly dementia patients.
