本研究為探究國内結束感化教育後續追蹤服務之實務現況，運用問卷調查方式，以全國提供服務之39家單位為研究對象，有效回收35家問卷，有效回收率為90.0%。研究結果顯示：服務方式以電話、家訪與網絡為主；服務内容以情緒支持與陪伴、工作尋找、就學輔導為主；執行困境包括個案缺乏改變、接受服務意願低、關係建立不易、個案負荷量高、缺乏成效指標、機構服務項目有限、就業市場限制等議題，可作為後續政策檢討之參考。研究建議：一、研發實務服務指引；二、強化機構專業督導系統；三、設計核心能力本位訓練；四、發展證據為本服務；五、整合後續追蹤服務資源。

The purpose of this research was to explore the practice and challenge of follow-up service for young offenders leaving reformatory education. A questionnaire survey was used to collect data from 35 organizations. The results show that: (1) service methods are mainly based on phone calls, visiting homes and Internet;⑵ main service items include emotional support, job search, school guidance; (3) difficulties in practice include lack of changes and cooperation, building relationships, high case loading, lack of outcome indicators, limited categories of services, and restriction of employment market.
