目的：運用分析系統，評估風險係數，針對高風險之系統，採取預防措施，提升醫院安全之執業及就醫環境。方法：透過危害分析評估環境中潛在失效原因及影響因素，擬定改善行動：舉辦教育訓練增進人員對環境認知、情感及信念；配合設備系統整合，制定設備維護制度，定期偵測安全管理稽核作業。結果：風險優先係數前測總分為2,595分，採取改善行動後降至479分，整體下降成效達81.5％，追蹤安全管理完成率為99.79%－100%，效果維持為99.9%。結論：運用FMEA結構性系統分析方法，發現環境中危害因子，採取預防性風險管理措施，確保醫院安全且零風險之環境。

Purpose: The aims of this study were to enhance the safety environment in a hospital. We appliedfailure mode and effect analysis (FMEA) to evaluate risk factors and took precautions in high-riskprocedures.Methods: Hazard analysis was employed to assess the potential causes of environmental failureand impact factors. On the basis of these factors, actions were taken, for example launching oftraining to promote environmental awareness, emotion, and belief; the integration of equipmentsystems; the development of an equipment maintenance system; and regular review of the auditwork of security management.Results: Before risk priority coefficient determination, we received a test score of 2595 points. Afterimprovements, the test score was 479 points and our overall decline in effectiveness was 95%. Ourtracking safety management completion rate was 99.79%–100%, and the effect was maintained at99.9%.Conclusions: Environmental risk factors were identified using FMEA structural system analysis, andpreventive risk management measures then ensured that our hospitals were deemed to be safe andwere zero-risk environments.
