利用前列腺動脈微球栓塞手術(prostatic arterial embolization, PAE)是近年來治療良性前列腺增生(benign prostatic hyperplasia, BPH)引起的下泌尿道症狀(lower urinary tract symptoms, LUTS)有效且安全的方法之一，目前雖然沒有如外科手術來得明顯有效，但是術後病患卻可以快速恢復，且生活品質大幅改善。本研究自107年10月至今共計50位求助患者，本研究自徺動脈透過微導管經前列腺動脈利用微晶球栓塞前列腺，阻斷血液造成前列腺體萎縮，並配合甲型抑制劑及相關藥物，協助病患改善排尿狀況，減少下泌尿道症狀。治療的患者中，單側治療共8人占比10%，其餘皆為雙側治療，術後皆無大量出血的併發症，不論單側或雙側的栓塞都可提升患者的尿流速，且提升生活品質，此外，有10位的心血管疾病患者正在服用抗凝血劑，經臨床評估不適宜接受傳統手術治療，術後一樣順利恢復，無產生任何副作用及併發症，但目前此治療術式並不在全民健康保險的給付範圍內，術式需高的費用，期望在日後可累積更多案例，能造福更多患者。

Prostate arterial embolization (PAE) with prostate arterial microsphere embolisation is one of the effective and safe methods for treating lower urinary tract symptoms caused by benign prostatic hyperplasia in recent years. Although there is no obvious effective surgery, the patients can recover quickly and the quality of life is greatly improved. We had a total of 50 patients seeking help since October 2018. We used the microcatheter through the microarticular artery, and block the blood to cause atrophy of the prostate, and cooperate with the alpha blockers inhibitor to restore the patient smooth urination. Among these treated patients, unnatural treatment accounted for 8% of the total, and the rest were bilateral treatment. There were no complications of massive bleeding after surgery. Both unilateral and bilateral embolization could improve the patien’s urine. The flow rate and the quality of life are improved, but at present the treatment is not in the scope of universal health insurance, the self-pay amount is higher, and we expect to accumulate more cause in the future, which will benefit more patients.
