本研究開發利用血壓脈波訊號系統裝置偵測到血管的變化,並且採用即時運算方式,以不改變使用者習慣的量測模式,達到更加有效率的血管與血壓監控。心血管疾病(Cardiovascular diseases, CVD)造成全球每年將近一千八百萬人口死亡,儼然成為世界衛生組織(WHO)近年來關注的一項重要議題,許多相關研究開始探討著有關CVD 的風險評估,其中總動脈順應性(Total Arterial Compliance, )被證實不僅能做為CVD 風險評估之外,還能夠做為分級預測進展之用,能有效的診斷心臟功能與負荷。本研究之自動化量測動脈順應性指數之裝置,採非侵入式肱動脈量測方式,利用示波法(Oscillometric Method)與恆壓法(Constant Pressure Method)提取血壓脈波波形,除了能夠獲得收縮壓(Systolic Blood Pressure, SBP)、舒張壓(Diastolic Blood Pressure, DBP)和平均血壓(Mean Blood Pressure, MBP)之外,還能透過脈搏舒張期分析,估算動脈順應性指數(Compliance Index, aC)。研究利用開發之系統收集了十五例健康者之數據,以浸泡約攝氏十度的冰水使血管短時產生收縮反應,以達到動脈順應性發生劇烈變化,此研究結果可發現動脈順應性指數aC與CT有直接的關係。

英文摘要

According to World Health Organization (WHO), cardiovascular diseases (CVDs) caused18 million people death each year. There are many indicators which can be considered for risk assessment, especially the total arterial compliance (CT) that has been shown to be a valuable index technique not only in CVD risk assessment but also in stratification and progression prognosis. Arterial compliance can be used to evaluate the relationship between structural and functional changes in the vascular system. In this study, we developed a non-invasive device using the oscillometric and constant pressure method for automatically real time analysis. The systolic blood pressure, diastolic blood pressure and mean blood pressure in the envelop of the waveforms were acquired from the envelop of the wave forms, and the index of arterial compliance was evaluate from the waveforms recorded at a constant pressure. Fifteen young volunteers were included and asked to immerse their feet into a cold water lasting for 10 minutes in order to cause a short-term vascular contraction and to alter the index of arterial compliance. The results showed that the aC,an index of arterial compliance, was highly correlated with the total arterial compliance CT.Therefore, the proposed non-invasive device may be employed to evaluate the total arterial compliance in homecare or clinical settings.
