背景與目的:我國物理治療專業發展至今已逾一甲子,專業領域發展已漸趨多元化,但職場條件卻每況愈下。從畢業生考照率低,就業者的求職困難,在職者的薪資下降與工作壓力大,這些問題都嚴重影響基層治療師的士氣,更導致整體基層治療師對專業發展的不確定與挫折感。然而回顧過去文獻,多是以人力需求、職業傷害、職業異動為考量。而基層治療師的勞動條件、生涯規劃、專業展望等因素,幾乎未受重視。現今適逢長期照護領域逐漸成熟,六年制物理治療學程開辦,過去研究的結果已跟不上專業趨勢的變化。因此本研究希望以基隆市從業治療師為例,調查臨床第一線治療師的執業現況與職涯規劃。方法:本研究於2018 年5月以基隆市物理治療師為對象,採不具名問卷調查進行。結果:回收個案量共73 人占所有基隆市治療師整體92.4%。研究分析顯示目前從業治療師平均年齡為34.7 歲,以女性占多數 (65.8%)。平均每名治療師約需經過1.8次考試即可考取證書,並未高於公務員高考的2.4 次,因此可推測國考通過率不佳並非難度過高,主因應為重複報考的比例過高所導致。所有治療師年收入之中位數及平均數為60 ~ 64 萬元,與過去相比下已實質衰退。影響薪資收入的決定因子為「從事兼職」、「正職固定調薪」、「投入治療所」與「週末上班」。目前所有治療師平均每週工時為38.8 小時,而工作中約僅八成時間投入臨床工作,每日平均治療人次為36.7 人次,整體治療負荷量已達2004 年的1.7 倍。結論:第一線治療師若謀求更高的收入時,則需投入更多時間以謀求獲益。原本預計為專業新藍海的長照領域,現行以兼職為主的模式已逐漸達到運作上限,然而不穩定的長照個案量影響了治療師投入的意願。

Background and Purpose: The development of physical therapy profession has grown for more than 60 years in Taiwan with increasing diversity. However, unfavorable working conditions such as low pass rate for the physical therapist licensure exam, difficultly of job searching, and working under great stress would seriously affect morale of clinical physical therapists, resulting in uncertainties and frustration. Topics such as manpower, work-related injury, and job turn-over which was published popularly in the past were unable to reflect the professional trend without surveying for working condition, career planning in this new era. The purpose of this study was to investigate working conditions and career planning of clinical physical therapists in Keelung. Methods: The study subjects were physical therapists in Keelung City in May 2018 with anonymous survey. Results: Seventy-three responses (92.4% of total samples) were used for data analysis. Research analysis shows that the average age of current practitioners is 34.6 years old, with women accounting for the majority (65.8%). On average, each therapist needs to take about 1.8 exams to obtain their certificate, which is no more than 2.4 times for the civil service senior examination. The median and average annual income of all therapists is NT$ 600,000 to NT$ 640,000, which is a real decline when compared with the past. The factors affecting annual income are “part-time job”, “fixed salary adjustment”, “working in physical therapy facility”, and “work on weekends”. At present, the average working hours of all therapists is 38.8 hours, while 80% of the time is clinical work. The daily productivity is 36.7 person-times, and the overall therapeutic load has reached 1.7 times than that of 2004. Conclusions: Although the current part-time-based model has gradually reached the upper limit, in the estimation of long-term care professional demand, the unstable source of clients has affected the willingness of the therapist to involved in this area.
