背景:血液透析病人必須仰賴透析治療才能將吃進來的「高磷」與「高尿酸」排除,研究證實單一高磷酸血症或高尿酸血症,都將引發心血管疾病,嚴重則會增加死亡率;但是否果真如此?目的:探討血清磷合併尿酸濃度對血液透析病人存活的影響。方法:採次級資料分析;取血液透析病人之血清磷與尿酸數值相乘,將依其乘積區分為低、中、高3組,比較各組對存活的影響;觀察期間為54個月。結果:共100位血液透析病人;第1組(乘積低)有34人;第2組有31人;第3組(乘積高)有35人。血清磷與尿酸濃度呈正向線性關係(r =0.53, P < .001)且「年齡」為影響病人存活之重要因素。以第3組病人為最年輕及高數值的身體質量指數、常態蛋白質分解率與白蛋白,達統計上顯著意義(P < .05 )。結論:血清磷合併尿酸濃度乘積高的血液透析病人,因為整體狀況為最好,故存活率為最佳。
Background: Hemodialysis patients must rely on dialysis treatment to eliminate "high phosphorus" and "high uric acid" in food. Many studies have confirmed that a single hyperphosphatemia or hyperuricemia will cause cardiovascular disease, and severely increase mortality. However, Is this true? Purpose: To investigate the effect of serum phosphorus combined with uric acid on the survival of hemodialysis patients. Methods: Analysis of secondary data. According to the serum phosphorus multiplied by uric acid, and then divided into low, medium and high according to the value, a total of 3 groups. Then, compare the survival relationship of the 3 groups of hemodialysis patients. The observation period was 54 months. Results: A total of 100 hemodialysis patients were included in the study. There are 34 in Group 1 (low product), 31 in Group 2, and 35 in Group 3 (high product). Serum phosphorus and uric acid concentrations show a positive linear relationship (r = 0.53; P < .001). In addition, "age" is the most important factor affecting patient survival. The 3 group is the youngest and highest values of body mass index, normalized protein catabolic rate, and albumin, which is statistically significant (P < .05). Conclusion: Hemodialysis patients with high serum phosphorus and uric acid concentration have the best survival rate because the overall condition is the best.
