本文描述一位男性肝癌末期患者正準備享受無壓力的退休生活時卻因腹痛及呼吸困難至醫院求治，診斷是肝癌末期，不僅使個案受到嚴重打擊情緒低落，對家屬更是帶來衝擊。筆者於2017年6月1日至6月26日照護期間，運用生理、心理、社會及靈性之健康評估，透過觀察、會談及身體評估等方式收集資料，確認個案有體液容積過量、呼吸道清除功能失效、疼痛、無望感等護理問題。筆者於護理過程中藉由正確的疼痛評估、與個案討論非藥物之疼痛緩解方法，如放鬆療法、轉移注意力，協助個案克服疾病導致生理的不適。經與個案及家屬共同擬訂具個別性及整體性之護理計畫，了解個案心理感受及想法，協助個案接受罹病事實，鼓勵家庭支持、引導宗教信仰的介入，使個案能以正向態度來維持生存希望。期望藉由此護理經驗之分享能夠提供護理同仁往後照護類似個案之參考。

A man, who was preparing to enjoy his carefree retirement, unexpectedly was suffered from the abdominal pain and breathing difficulties to take the medical treatment. Further astonished, he was diagnosed as a terminal liver cancer patient. Not only was the patient severely shocked, but also his families were hit by more impacts. During the care period, from 01 June to 26 June in 2017, we collected the patient’s relevant data through observation, interviews and physical assessment by applying physical, psychological, social and spiritual health assessments. The patient was confirmed with numerous nursing problems, such as: excess body fluid volume, failure of respiratory clearing function, pain, and hopelessness, etc. In order to assist the patient could overcome his physical discomfort caused by the disease, we discussed non-drug pain nursing relief solutions with the patient by means of correct pain assessment, such as: relaxation therapy and attention shift. Then, we jointly drafted the customized and integral nursing care plan with patient and his families. The purpose of enabling the patient could survive with the positive attitude, we should realize patient’s psychological affections and ideas, should assist patient to accept the facts of rickets, and encourage his families’ utmost supports as well as the intervention of religious beliefs. Wish our nursing care experience could establish the further references for nurses caring on the similar patients.
