個案因中風吞嚥困難及右側肢體無力，擔心拖累家人，出現負向情緒。筆者在2016/5/30至7/28以觀察、陪伴及身體評估等方式，運用整體性護理評估，確立個案有吞嚥功能障礙、身體活動功能障礙及無力感等健康問題，藉由腦中風急性後期之專業跨領域團隊合作擬定個別性照護及復健計畫，執行吞嚥訓練運動及飲食指導，降低嗆咳及吸入性肺炎之危險性；進行高強度復健治療，改善右側肢體無力，恢復自我照護能力。另外也以社會支持及家人陪伴方式，讓個案接受身體吞嚥及身體活動功能障礙所帶來的衝擊，進而以正向及積極的態度面對長期復健活動。希望藉此經驗分享，提供照顧腦中風病人之參考。

Due to stroke, the patient had dysphagia and right limb muscle weakness. Worrying that he was a burden on his family, he had negative emotion. During the nursing period from May 30 to July 28, 2016 the researcher employed the ways of observation, companion and health assessment, and used overall nursing assessment to establish firmly that the patient had the health problems of swallowing dysfunction, impaired physical mobility and feeling of physical powerlessness. Through collaboration of professional cross-field teams in curing post-acute stroke, individualized caring and rehabilitation plan was drafted. Swallowing exercise and dietary guidelines were offered to the patient in order to reduce the danger of choking, coughing and getting aspiration pneumonia. High-intensity rehabilitation treatment was carried out, intending to improve the patient’s right limb muscle weakness and to resume his self-care ability. Besides, with social support and family companion, the patient was made to accept the impacts brought by dysphagia and impaired physical mobility. Furthermore, the patient took a positive attitude to face the long-term rehabilitation activities. It is hoped that through sharing of this nursing experience, the process can be provided as a reference for people in taking care of stroke patients.
