背景:手臂振動症候群(HAVS)的定義是振動從工具傳遞到工人的手和手臂。長時間暴露於由手傳遞而來的振動會導致上肢的血管、神經系統和骨關節系統疾病增加。一名工人來到職業醫學門診,欲獲取腕隧道症候群的職業病證明。我們因此有機會評估該疾病與職業之間的關係。案例介紹:某一疑似HAVS症狀的患者被轉介到我們的職業病/傷害門診。著手進行調查後發現該個案多年來因工序暴露於高頻率振動,進而發現多名工作同仁也患有振動引起的神經肌肉病變的個案系列。我們介紹了一個在金屬研磨工藝部門加工廠工作20年的工人的案例,他在2013年被診斷出患有雙側雷諾現象和腕隧道症候群。此外,我們也對他的同事進行調查以評估職業促發疾患的群聚狀況。研究方法與風險評估:我們收集了作業者的工作經歷和個人經歷,並進行了完整的神經電生理檢查。8名工人中有7名的工作時間為15至20年。8名工人中有6名被診斷患有腕隧道症候群。結果:我們證實當前案例系列中的HAVS及腕隧道症候群與暴露於研磨過程中的高頻率振動有關。建議使用適當的個人防護設備以防止這類職業病的發生。

英文摘要

Background: Hand-Arm Vibration syndrome (HAVS) is defined as the transfer of vibration from a tool to a worker’s hand and arm. Prolonged exposure to hand-transmitted vibration is associated with an increased occurrence of disorders in the vascular, neurological and osteoarticular systems of the upper limbs. A worker came to the out-patient department of occupational medicine for occupational disease certification of carpel tunnel syndrome. We would like to evaluate the relationship between the HAVS and occupation. Case presentation: We present a case of a worker who had worked in the grinding process department of a metal processing factory for 20 years. He was diagnosed with bilateral Raynaud’s phenomenon and carpal tunnel syndrome in 2013. Furthermore, we investigate his co-workers to evaluate the condition of clustering. Methods and risk assessment: We collected the working history and personal history of the workers and conducted complete neuro electrophysiological exams. 7 out of 8 workers with working duration from 15 to 20 years. Six out of 8 workers were diagnosed with carpal tunnel syndrome. Conclusions: We confirm that HAVS as carpal tunnel syndrome was associated with exposure to high frequency vibrations during grinding process in the current case series. Proper personal protective devices were suggested to prevent the occurrence of such occupational diseases.
