乳房攝影立體定位的真空輔助切片是針對摸不到的可疑病灶,且具有微細鈣化點的部分來進行切片化驗的方法之一,其最常使用的進針方式是採用垂直方向的進針;但由於部分患者乳房固定壓夾的厚度太薄(小於2 cm),或是鈣化的位置太過表淺,以至於太靠近皮膚,因此,無法使用垂直進針方式來進行切片。為了克服無法垂直進針的難題,本研究目標發展由側位進針的切片模式。本研究進行18位無法垂直進針的臨床個案收案。在結果中,側位進針的切片模式除了可以提升進行粗針切片的可行性,也降低進開刀房執行傳統開刀取出微細鈣化做化驗的方式。本研究為臨床乳房攝影導引切片之模式進行改良,為後續臨床應用開啟一扇新窗。
Stereotactic vacuum-assisted breast biopsy in mammography diagnoses nonpalpable lesions with microcalcifications. Vertical needle insertion is the most frequently used method in breast biopsy systems recently. However, some patients’ breasts are too thin, which are less than 2 cm, while their breasts are compressed, and the vertical needle insertion method is not applicable in this situation. In this study, we introduced an adopted method with a needle from lateral approach. We carried out eighteen clinical cases who were not able to insert a needle by vertical entry. In the result, we demonstrated the mode of lateral approach was not only improved the feasibility of biopsy applying, but also reduced the way of operating surgery to take out microcalcification. The lateral approach conclusively could be an optional method used for clinical stereotactic vacuum-assisted breast biopsy in the future.
