本研究目的探討在肘關節常規攝影中,肘關節外科手術後以石膏固定之患者如何在有限擺位角度下,參考側面肘關節石膏固定角度對應X光中心射束入射角度來達到前後投射影像呈現出較佳的影像品質,並改善受檢患者的配合度與舒適度。從2018年10月迄2020年4月收集100例骨科病房針對術後肘關節以石膏固定之患者採用站姿及躺姿的方式攝影追蹤術後受檢者肘關節情況,與收集100例門診X光肘關節石膏固定,以及100例一般常規攝影受檢者,並安排兩位放射線專科醫師及一位年資具30年資深放射師調整X光入射角度方式所得到的影像來評估兒童、青壯年及老年的受檢者肘關節X光攝影接近於正常肘關節延展時的標準解剖影像,依此方式應用於骨科病房案例作報告,並請放射線專科醫師診斷位置比對作評分。調整射束角度後的實驗結果得知,影像可作為臨床醫師診斷參考。此實驗先測量側面肘關節以石膏固定的外側角度夾角的1/2角度作為前後位投影照射參考角度,並評估在患者肘部石膏外側作夾角角度量測,與X光照射後所得側面肘部影像外側夾角分析,其結果夾角誤差值在0-5度與6-10度比較其差異性,得知結果有顯著差異,佐證藉由側面肘關節夾角來調整前後位攝影入射角度的可行性,並藉由此實驗探討以1/2入射角度為最適化。

英文摘要

The purpose of this study is to investigate how to fix the angle of plaster fixation on the side of the elbow joint with the angle of incidence of the X-ray center beam for the anterior posterior projection images of patients with gypsum fixation after elbow surgery in conventional photography of the elbow joint Presents better image quality and improves the fit and comfort of the patient under test. From October 2018 to April 2020, 100 orthopedic wards were collected. Patients with postoperative elbow joints fixed by plaster were photographed to track the elbow joints of the postoperative subject and 100 outpatient X-rays were collected. Elbow joint gypsum fixation and 100 general routine photography subjects, and arranged two radiologists and a 30-year senior radiation radiographer adjust the X-ray incident angle to obtain images to evaluate children, young and old The subject's elbow joint X-ray photography is close to the standard anatomical image of the normal elbow joint extension. Apply this method to report cases in orthopaedic wards and ask the radiologist to compare the diagnostic position for evaluation. After adjusting the beam angle, the experimental results show that the image can be used as a diagnostic reference for clinicians. In this experiment, the lateral elbow joint is measured with plaster. The half angle of the lateral angle of the angle is used as the reference angle of the front and rear projection irradiation, and the angle measurement is measured on the outside of the patient's elbow plaster, and the lateral angle of the elbow image obtained after X-ray irradiation is analyzed. The resulting angle error the value is compared between 0 to 5 degrees and 6 to 10 degrees, and the results are significantly different. It proves the feasibility of adjusting the angle of incidence of anterior posterior photography by the angle of the side elbow joint, and through this experiment, the dichotomy one of the incident angles is optimized.
