2020年1月,世界開始爆發嚴重特殊傳染性肺炎(Severe Pneumonia with Novel Pathogen),血液透析病人需長期接受透析治療且進出醫院頻繁。一旦有感染新興傳染病,若未適當採取保護措施,易爆發群聚感染。因此,本血液透析單位啟動應變因應,全面調整及修訂單位的內部作業規範。所訂定的風險監控重點涵括1.人員管理:人員排班分艙分流、人員標準配備規劃;2.人員教育:提升護理人員及民眾防疫防護認知、落實體溫監測與手部衛生、教育訓練計劃安排;3.環境控制:環境設備清潔、機器清潔及防疫物資整備。分享本透析單位防疫成功的經驗、協同團隊整合防疫內部規範,期能共同隨時迎戰嚴重特殊傳染性肺炎,這才是防疫成功的重要關鍵。

英文摘要

In January 2020, an epidemic pneumonia caused by the novel coronavirus (COVID-19) broke out in the world. Hemodialysis (HD) is mostly used to treat patients with end-stage renal disease, which require long-term treatment and frequent medical visits. Once a HD patient infected with an emerging infectious disease such as COVID-19, clusters may occur in the dialysis unit if appropriate infection prevention and infection control measures are not taken. To ensure the safety of the medical staff and the patients, and to prevent the occurrence and spread of infectious events, the HD Unit initiated a series of response procedures. Our hemodialysis unit updated and revised the internal regulation and measures in response to the COVID-19 epidemic abiding by the “Guidelines for infection control in medical institutions responding to COVID-19 infection” of the Taiwan Centers for Disease Control (CDC), the “Hemodialysis Room Infection Control & Management Procedures” of the Kaohsiung Medical University Hospital (KMUH), and the “KMUH Staff Pandemic Screening Principles”. Our key procedures include personnel management (focusing on patient triage, patient allocation, and dividing and fixed care group), personnel education (to enhance knowledge and awareness to the COVID-19 pandemic of the medical staffs and the patients), and environmental control (stringent disinfection and sanitization of equipment & machinery are mandatory). We have a good interim result regarding the infection prevention and infection control to the COVID-19 pandemic in Taiwan. We share our HD Unit’s experience and policies during this period. Through mutual learning, cooperation, and teamwork, the medical system can better prepare and response to any future challenges of novel infectious disease.
