依據衛福部死因統計,每年約13,000人死於慢性肝病、肝硬化及肝癌(hepatocellular carcinoma,簡稱HCC),B型肝炎帶原者如持續有肝炎的發作,有15-20%會發生肝硬化,大幅增加肝癌的發生率,而肝硬化患者每年有3-5%會變成肝癌。Gd-EOB-DTPA為一種具肝臟特異性的磁振造影對比劑,和傳統對比劑最大的不同是顯影方式和代謝的方式。本病例報告主要是探討Gd-EOB-DTPA增強的磁振造影檢查和電腦斷層檢查兩者對於小肝腫瘤的診斷效能,特別針對於小於2 cm的HCC有極佳的鑑別度,提供肝癌患者在醫院進行檢查的另一種參考選擇,不僅可協助醫師提高的診斷行為,並減少病患額外的醫療浪費和時間,大幅提高就醫品質,未來應用在肝癌病患的照護保健與肝功能研究應具參考價值。

英文摘要

On the basis of MOHW causes of death statistics, approximately 13,000 people die from chronic liver disease, liver cirrhosis and HCC every year. If HBV carriers continue to have hepatitis attacks, 15-20% will change into liver cirrhosis with increased chance of liver cancer. Patients with liver cirrhosis 3-5% will change into liver cancer. Gd-EOB-DTPA is a liver-specificity MR contrast medium with biggest difference from traditional contrast medium in image enhancement and metabolism. This case report mainly discusses the difference between Gd-EOB-DTPA enhanced magnetic resonance imaging and computed tomography in the diagnosis of small liver tumors. Gd-EOB-DTPA enhanced magnetic resonance imaging gives excellent diagnostic performance for HCC smaller than 2 cm, providing another reference option for liver cancer patients to be examined in the hospital. It can not only help physicians improve their diagnostic behaviors, but also reduce patients’ extra medical waste and time, significantly improve the quality of medical treatment. The future application in the care of patients with liver cancer and liver function research should have reference value.
