毛毛樣血管疾病是一罕見腦血管疾病,其特徵是兩側內頸動脈遠端及前、中大腦動脈近端發生漸進式狹窄阻塞,並因補償作用於基底核區產生廣泛性網狀側枝循環,在腦血管攝影的影像下看似“煙霧”狀般。腦部磁振造影中血管影像序列是以時間飛躍技術利用流入增加效應進行血管成像,不需注射對比劑,能快速的檢查顱內血管的結構,進而組成影像。當然數位減贅血管攝影對腦血管狹窄閉塞的評估是重要標準,但具侵有襲性和注射對比劑的風險。本次報告是一位29 歲女性患者兩周內多次頭痛,並伴隨左側半邊麻痺無力,磁振造影幫助快速診斷出疑似毛毛樣血管疾病,配合數位減贅血管攝影確定血管狹窄和閉塞程度,使得治療有良好的評估影像。毛毛樣血管疾病經過治療後磁振造影可持續追蹤毛毛樣血管疾病的改善情況。
Moyamoya disease is a rare cerebrovascular disease. It is characterized by progressive stenosis and obstruction of the distal ends of the internal carotid arteries on both sides and the proximal ends of the anterior and middle cerebral arteries, and due to the compensation effect on the basal nucleus area, it produces extensive reticular collateral circulation, and the image of the cerebrovascular is like "smoke". The image sequence of brain magnetic resonance angiography is based on the time of flight to use the increased inflow effect to perform blood vessel imaging, without contrast medium, and quickly check the intracranial blood vessels. Digital subtraction angiography is an important criterion for the assessment of cerebral vascular stenosis and occlusion, but it is invasive and has the risk of contrast injection. The report is a 29years old female patient with multiple headaches in two weeks with paralysis of the left side and weakness. Magnetic resonance imaging for rapid diagnosis of suspected hairy vascular disease, digital angiography to determine vascular stenosis and occlusion, so that treatment has a good image. After treatment of moyamoya disease, magnetic resonance imaging can continue to track the improvement.
