背景與目的:遠端橈骨骨折復位處理後常導致水腫、疼痛、握力下降、關節僵硬變形,影響手部功能及日常生活功能獨立等。本研究目的在了解早期採用居家運動方案與傳統職能治療,相較於僅接受傳統職能治療,分析骨折後2、4及12週,在各項負向症狀的改善成效。方法:採方便取樣,於南部某區域教學醫院招募30位遠端橈骨骨折個案,隨機分派至介入組和控制組。介入組於骨折後2週開始進行為期兩周之居家運動衛教方案,此段時間內控制組則僅以門診口頭衛教;兩組均於石膏固定移除(骨折後4週)後接受職能治療。於受傷後第2、4及12週評估關節活動度、握力、疼痛、水腫、肩部肌耐力、上肢及手功能表現及生活品質。資料分析包括描述性統計及混合型共變數分析。結果:兩組在關節活動度、握力、水腫、疼痛、肩部肌耐力、生活品質等皆有不同面向的顯著組間差異存在(p<.05);介入組在大多評估變項中一般皆有較好的恢復趨勢。結論:研究結果支持職能治療結合2週早期居家運動方案可以幫助遠端橈骨骨折患者促進患側上肢手功能恢復並提升個案傷後生活品質。

英文摘要

Objectives: The distal radius fracture after reduction and fixation often leads to sequelae such as edema, pain, decreased grip strength, and joint stiffness, which affect clients’ hand function and independent living in daily activities. The purpose of this study was to analyze the difference of treatment outcomes at 2nd, 4th and 12th weeks between occupational therapy (OT) treatment combined with early home-based exercises program and OT treatment with outpatient oral home education program. Methods: Convenience sampling was used to recruite 30 participants with distal radius fracture from a local teaching hospital at southern Taiwan. The participants were randomly assigned into either an intervention or a control group. At 2 weeks post-injury, the intervention group underwent early home-based exercise and the control group received oral home education program for 2 weeks. Afterwards, both groups received conventional OT treatment at 4 weeks post-injury. Participants’ joint range of motion, muscle strength, endurance, pain, edema, hand function, and quality of life were assessed at the 2nd, 4th and 12th weeks. Descriptive statistics and mixed analysis of covariance (ANCOVA) were used for data analysis. Results: The intervention group had significantly more improvements (p < .05) than the control group in joint range of motion, grip strength, edema, pain, shoulder endurance, and quality of life. Conclusions: Our study supported that occupational therapy combined with a 2- week early home-based exercise program could enhance functional recovery of the affected upper limb and increase quality of life in participants with distal radius fracture.
