前言:根據世界衛生組織報告,失智症發生率隨著年齡增長而增加,每年世界會增加990萬新個案。我國衛生福利部積極推動失智照護資源佈建,但現行社區失智照顧單位,提供認知及日常生活功能促進量能較為不足,部分極輕度失智者更期待從事有產值的活動。參考友善商家一日工作坊、上錯菜餐廳等經驗,本研究協助受試者參與工作坊,了解其對極輕度失智者之益處。方法:共納入3名極輕度失智者(臨床失智評估量表=0.5)參與研究,完成為期共11週的失智友善工作坊。受試者須完成基本資料收集、前後測驗,其中測驗項目包括簡短心智測驗、工具性日常生活活動能力量表、工作滿意度。結果:三位受試者的原始簡短心智測驗分別為26分,23分以及23分。在經歷工作坊經驗後,簡短心智測驗進步至27分,25分以及29分。三名受試者於工具性日常生活活動能力前分別為18分,16分以16分、後測進步至21分,17分以及17分,而於上街購物、服用藥物、處理財務及外出活動領域,有增進獨立性趨勢。結論:經由參與工作坊經驗,發現極輕度失智者的部分認知功能、工具性日常生活能力,有改善的趨勢。為了解參與工作坊之好處,未來需要更多失智者共同參與工作並且持續追蹤。

英文摘要

Introduction: According to the World Health Organization, incidence of dementia increases with higher age, and there are 9.9 million new cases with dementia worldwide every year. The Ministry of Health and Welfare in Taiwan actively promotes dementiacare resources. However, the existing community dementia-care units are unable to provide adequate support for cognitive and daily living functions, and some persons with very mild dementia prefer engaging in productive activities. Drawing from the experiences of “The Restaurant of Order Mistakes” and “Friendly Stores One-Day Workshop”, this study explored the benefits of attending sheltered workshop program for persons with very mild dementia. Methods: Three women with very mild dementia (Clinical Dementia Rating = 0.5) participated in an 11-week dementia-friendly workshop. They completed pre- and posttests including the Mini-Mental State Examination (MMSE), Instrumental Activities of Daily Living (IADL) scale, and job satisfaction. Results: The baseline MMSE scores of the three participants were 26, 23, and 23, and the post-test scores were increased to 27, 25, and 29, respectively. The IADL pre- and posttests showed mild improvement for all participants (pre-tests: 18, 16, 16; post-test: 21, 17, 17), with increased independence in the areas of shopping, medication, financial management, and activities outside the home. Conclusion: The participants with very mild dementia seem to improve their cognitive functions and IADL skills after the sheltered workshop program. Future research should validate the benefits of sheltered workshop program with a large sample size and long-term follow-up.
