本文描述協助ㄧ位因毛毛樣血管疾病導致中風個案脫離呼吸器之護理經驗。筆者於2018/9/12至2018/10/5護理期間，經由觀察、會談、嘴型表達與身體評估等方式收集相關資料，並以Gordon十一項功能性健康型態進行評估，發現個案有呼吸機戒斷反應功能失常、自我照顧能力缺失與焦慮等護理問題。照護期間指導個案正確咳嗽技巧和吸氣肌肉訓練，改善痰液排除與促進呼吸肌力，維持呼吸道暢通；衛教並鼓勵執行自我照顧能力技巧，設計適合個案之復健活動，提升其日常生活活動能力(ADL)，並透過主動陪伴、引導個案抒發感受，運用音樂治療轉移不愉快之情緒困擾，減輕焦慮，最後使個案順利脫離呼吸器。期望藉此護理經驗分享，提供護理同仁照護此類個案之參考，提升護理專業之照護品質。

This is the nursing experience of a stroke patient caused by Moyamoya disease who underwent weaning from a mechanical ventilator is presented. The author collected relevant information from observation, conversation, lip language, and physical assessment with Gordon's functional health patterns, during September 9, 2018 to October 5, 2018. It was assessed that the patient had nursing problems such as dysfunctional ventilatory weaning response, self-care deficit and anxiety. During the nursing process, to begin with, the patient was taught to generate effective coughing and respiratory muscle training to facilitate sputum removal, rebuild respiratory muscle strength and maintain airway patency. Instructions and encouragements were provided to create self-care skills and appropriate rehabilitative activities into practice for the enhancement of Activities of Daily Living (ADL). Furthermore, music therapy, accompanying and encourage expressing the patient’s innermost feelings could alleviate the patent’s anxiety from emotional disturbance. Finally, this patient was successfully weaned from a mechanical ventilator. We would like to share our nursing experience and expect this to be used as a reference when providing nursing care for these patients, also to improve the quality of nursing care.
