背景:為了解花蓮縣室內裝潢木工業勞工的健康狀況、工作環境危害因子以及職業因素造成之聽力及肺功能危害。方法:花蓮慈濟醫院東區職業傷病防治中心進行勞工健康狀況及職業暴露之問卷調查、肌肉骨骼症狀調查、預防性聽力篩檢、預防性肺功能篩檢,並進一步,將勞工聽力健康分級管理與勞工自覺症狀以及作業經歷進行交叉比較;藉由本次篩檢,提供篩檢勞工預防職業傷病之相關衛教、職業傷病諮詢服務,提高花蓮地區勞工之職業衛生危害認知以及危害預防。結果與討論:於106年08月01日至106年09月31日進行裝潢木工業勞工健康篩檢活動,篩檢地點為花蓮慈濟醫院職業傷病防治中心及花蓮縣木工工會,有效問卷共66份,肌肉骨骼調查顯示疼痛、酸麻的部位主要有下背(17.7%)、其次為右肩(14.6%)、左肩(12.5%);健康篩檢結果顯示肺功能異常者13人(19.7%)、聽力異常者47人(72.3%),初步排除其他影響因子後,肺功能異常群聚現象有7人(10.6%)、噪音造成之聽力損失群聚現象有16人(24.6%)。結論:於篩檢後提供聽力保護具、職業傷病預防宣導摺頁,對勞工進行衛教宣導、職業傷病諮詢服務,使其了解工作中肌肉骨骼、噪音及粉塵暴露危害以及勞工健康服務之目的;並於篩檢結束後,與胸腔內科醫師、職業醫學專科醫師進行篩檢結果探討,安排肺功能異常及聽力損失之勞工至專科進行健康檢查追蹤。

Background: To evaluate the occupational health hazards of upholstery workers in Hualien County, the hazardous factors of the work environment, and the musculoskeletal symptoms, hearing and pulmonary function were investigated. Methods: The Eastern Occupational Injury and Disease Prevention Center at Hualien Tzu Chi Hospital conducted questionnaire surveys on health status and occupational exposure, surveyed musculoskeletal symptoms, preventive hearing screening, preventive lung function screening. The correlation among the management level of noisy operation, personal perceived symptoms and work exposure were elucidated. Case managers of the center also provided relevant health education and occupational-injuries prevention consultation to promote the awareness of labors to occupational health hazards. Results and discussion: The health status screening for upholstery worker was conducted from August 1, 2016 to September 31, 2017 at the Center in Tzu Chi Hospital and the Carpenter Labor Union in Hualien County. The questionnaires of 66 cases were valid. The musculoskeletal symptoms survey revealed the major symptoms were lower back pain (17.7%), right shoulder pain (14.6%), left shoulder pain (12.5%). Pulmonary function test found 13 cases with abnormal lung function (19.7%), and pure-tone hearing test identified 47 cases (72.3%) with hearing loss. After preliminary exclusion of confounding factors, there were 7 cases (10.6%) with abnormal lung function and 16 (24.6%) with hearing loss. Conclusion: After the screening, the center provided personal hearing protection equipment and leaflets for prevention the health hazard of ergonomic, dusty and noisy operations to promote their awareness toward occupational hazards. The results of the screening were discussed with chest physicians and occupational safety specialists, and arrange those workers with abnormal pulmonary function and hearing loss for further physical check-up and follow-up.
