目的：依醫策會醫病共享決策的策略發展決策輔助工具，了解輪狀疫苗認同程度及施打比例。方法：以實證方式，使用PICO將關鍵字以布林邏輯做聯集及交集搜尋文獻，以CEBM之「Diagnostic Study Critical Appraisal Sheet中文版」評讀，用GRADE評估結果的確定性，依醫病共享決策的策略發展決策輔助工具，以問卷收集認同程度，以病歷調查口服率。結果：輪狀病毒疫苗口服率在使用前為43％增加至96％。使用決策輔助工具認同度以「幫助我認清到我必須做出決定?」分數4.28最高。使用決策過程認同度以「讓我更信任醫療人員對我的醫療處置」分數4.44最高。認知程度正確性76.9%-98.0%，以「輪狀病毒易感染6M~2Y幼兒」正確性最高，「疫苗可保護自己與他人」正確性最低。使用決策工具後選擇的信心指數80分以上為91%。結論：國內輪狀病毒疫苗無健保給付，故衛教家屬輪狀病毒疫苗的作用與預防，本案確認家屬在經過PDA的指導後，獲得家屬的認同，增加家屬認知，提升服用比率，也讓家屬選擇醫療決策更安心，使嬰兒免於輪狀病毒感染。

Objectives: To complete the Patient Decision Aids Tool. To compare the vaccination rate before and after using PDA. Methods: The empirical method was used in this study. Identified keywords for PICO. The keywords were used as a union and intersection through the Bollinger logic to search literatures. The literatures were strictly evaluated by CEBM's and use hazard ratio and 95% confidence interval to combine two sets of data. Then analyzed by national mortality, and used GRADE to assess the certainty of results. The evidence was sufficient to present the quality of the included studies. Results: The vaccination rate of rotavirus vaccine increased from 43% to 96% after using PDA. The average identification score of using PDA was 4.21 points. The average identification score to the use of decision-making process was more than 4.41 points. The confidence index of selection after using the decision tool were 91% more than 80 points. Conclusions: This case confirmed that the recognization to choose of family members can be helped after PDA guided. At present, there is no national health insurance for domestic rotavirus vaccine. Hope by this empirical report to make clinical nurses to get better understanding of the importance of rotavirus vaccines and furthermore promote the vaccines.
