本文為描述運用Swanson關懷理論照顧一位思覺失調症患者伴隨自殺行為之護理過程，個案長期在父親權威式的教育方式下，不敢表達自身想法，對於壓力因應多採隱忍、壓抑來面對，因中斷服藥造成病情變化，在與父親的衝突後出現負向想法與自殺行為，護理期間於2018年6月02至7月13日藉由簡式健康量表評估自殺危險性及定期會談、觀察等，發現個案有「潛在危險性自殺」、「個人因應能力失調」、「不遵從」之護理問題。筆者透過Swanson關懷理論中所提之的五大關懷核心，瞭解、在旁陪伴、為他做某些事、使他能夠與維持信念，協助個案克服生活中的壓力問題，改善負向想法、解除自殺危機。筆者發現此次運用Swanson關懷照護理論於臨床照護中，對於建立治療性關係有很大幫助，並且能協助個案建立正向的信念，期盼透過此次照護經驗分享，提供護理人員日後在面對照護類似個案之參考。

This case report describes the nursing experience that applied Swanson’s Caring Theory to a Schizophrenia patient with suicidal behaviors. The patient's personality was influenced by his father's authoritarian style. He was reluctant to share his thoughts and was prone to suppressing any stress he encounters. The patient's condition became unstable after he stopped oral medication without a doctor's prescription. Negative thoughts and suicidal behaviors often appeared after conflicting with his father. The nursing care was provided from June 2nd to July 13th, 2018. The patient's suicide risk was evaluated using the Brief Symptom Rating Scale along with regular interviews and observations. The associated health Problems such as Risk for suicide, ineffective coping, and Noncompliance behaviors were identified. The author applied the five step of Swanson’s caring theory：knowing, being with, doing for, enabling, and maintaining belief. The intervention included coping with stress, getting rid of negative thoughts, and resolving suicide risk. The author realised that this theory is effective in establishing a medical therapeutic rapport and leading the patient towards positive thoughts. It is hoped that this treatment experience could become a helpful reference for future cases.
